摘要
炎症性肠病的典型症状表现为胃肠道疾病,但也可能影响其他器官和系统,其中眼部表现是第三常见的肠外表现,主要涉及巩膜、葡萄膜、视神经及眼眶等病变,以及继发于炎症性肠病的眼部表现或治疗的白内障、青光眼、视网膜静脉阻塞或角膜病变等。治疗通常使用局部皮质类固醇或非甾体类药物控制炎症,而对于病情严重的患者可能需要全身使用类固醇或免疫抑制剂。炎症性肠病中的眼部表现是非特异性的,可以在肠道受累之前、同时或之后发生,临床医生可能有时无法意识到它们的相关性,及早发现并进行适当的干预对于眼部病情的进展至关重要。本文就国内外文献中出现的关于炎症性肠病眼部表现及治疗的最新进展予以综述。
Inflammatory bowel disease(IBD)is primarily a disease of the gastrointestinal tract,though it may affect other organ systems.Ocular manifestations are the third most common extraintestinal manifestations(EIMs),which mainly involves complications such as sclera,uvea,optic nerve and orbit,as well as cataract,glaucoma,retinal vein occlusion or keratopathy secondary to ocular manifestations or treatment of IBD.Topical steroids non-steroidal anti-inflammatory drugs are usually used to control inflammation in treatment,but systemic steroids or immunosuppressants may be necessary in severe cases.Ocular manifestations of IBD are commonly nonspecific and are not related to the time of intestinal involvement.Clinicians often fail to recognize their correlation.Early detection and appropriate intervention are essential to inhibiting the progression of eye diseases in IBD patients.This paper reviews the ocular manifestation and treatment of IBD.
