无创呼吸机在儿童重症肺炎治疗中的应用 被引量:4

Application of Non-Invasive Ventilator in the Treatment of Severe Pneumonia in Children

摘要 目的探究与分析在儿童重症肺炎治疗过程中应用无创呼吸机的效果及价值。方法将本院重症科于2017年3月至2019年3月收治的120例重症肺炎儿童患者纳入研究样本,采用随机数字表法分为有创组与无创组,各60例。有创组均予以重症肺炎的常规对症治疗联合有创呼吸机支持治疗;无创组均予以重症肺炎的常规对症治疗联合无创呼吸机支持治疗。观察并比较两组患者经治疗24h后的临床效果,治疗前后患者肺功能相关指标、血气分析相关指标及心率变化。结果无创组的总有效率为95.00%,有创组总有效率为80.00%,无创组显著优于有创组,差异具有统计学意义(P<0.05);经治疗后无创组肺功能、血气分析指标及心率改善程度显著好于有创组,差异有统计学意义(P<0.05)。结论无创呼吸机应用于儿童重肺炎中,其效果显著,对患儿肺功能、动脉血氧分压及动脉血二氧化碳分压等改善均有促进作用且无创伤,适合临床推广。 Objective To explore and analyze the effect and value of non-invasive ventilator in the treatment of severe pneumonia in children.Methods A total of 120 children with severe pneumonia admitted to our department from March 2017 to March2019 were enrolled in the study.They were randomly divided into the invasive group and the non-invasive group,60 cases each.Invasive group was treated with conventional symptomatic treatment of severe pneumonia combined with invasive ventilator support therapy.The non-invasive group was treated with conventional symptomatic treatment of severe pneumonia combined with non-invasive ventilator support.The clinical effects of the two groups of patients after 24 hours of treatment were observed and compared.The lung function related indicators,blood gas analysis related indicators and heart rate changes before and after treatment.Results The total effective rate was 95.00% in the non-invasive group and 80.00% in the invasive group.The non-invasive group was significantly better than the invasive group,and the difference was statistically significant(P<0.05).After treatment,the lung function,blood gas analysis index and heart rate improvement of the non-invasive group were significantly better than the invasive group,and the difference was statistically significant(P<0.05).Conclusion Non-invasive ventilator is effective in children with severe pneumonia.It has a significant effect on lung function,arterial oxygen partial pressure and arterial blood carbon dioxide partial pressure,and is non-invasive.It is suitable for clinical promotion.
