摘要
目的探讨温肾健脾法联合马来酸曲美布汀胶囊治疗脾肾阳虚型腹泻型肠易激综合征(IBS-D)的疗效以及对肠道菌群的影响。方法选择符合纳入条件的IBS-D患者88例为研究对象,按随机按数字表法分为观察组和对照组,各44例。对照组服用马来酸曲美布汀胶囊,1次2粒,每日3次。观察组在对照组治疗基础上采取温肾健脾法方药治疗,每日1剂,每日2次。连续治疗4周后,比较2组脾肾阳虚证症状评分、排便情况、临床疗效以及肠道菌群变化。结果治疗后,观察组脾肾阳虚证症状评分以及排便次数、粪便形状、黏液便评分明显低于对照组(P<0.01);观察组总有效率为95.45%,显著高于对照组的77.27%(P<0.05);治疗后,观察组双歧杆菌、乳酸杆菌显著高于对照组,大肠杆菌、肠球菌明显低于对照组(P<0.01)。结论温肾健脾法联合马来酸曲美布汀胶囊治疗IBS-D脾肾阳虚证的疗效确切,且可有效改善患者的肠道菌群。
Objective To observe the efficacy of Wenshen Jianpi Method combined with Trimebutin Maleate Capsule in treating irritable bowel syndrome with diarrhea (IBS-D) with syndrome of spleen-kidney yang deficiency and its effect on intestinal flora.Methods Eighty-eight IBS-D patients were selected,and randomly divided into observation group and control group by number table method,with 44 cases each.The control group was given Trimebutin Maleate Capsules,2 capsules once,3 times a day.The observation group was treated with warming kidney and spleen prescription on the basis of the treatment of the control group,one dose per day,twice a day.After 4 weeks’ treatment,symptoms score of syndrome of spleen-kidney yang deficiency,defecation score,the efficacy,and changes of intestinal flora were compared in the two groups.Results Symptoms score of syndrome of spleen-kidney yang deficiency,defecation frequency,stool shape,mucus stool score in the observation group were obviously lower than those in the control group after treatment (P<0.01).Total efficacy in the observation group was 95.45%,which was obviously higher than 77.27% in the control group (P<0.05).After treatment,bifidobacteria and lactobacillus in the observation group were significantly higher than those in the control group,and escherichia coli and enterococci were significantly lower than those in the control group (P<0.01).Conclusion Wenshen Jianpi Method combined with Trimebutin Maleate Capsule can treat IBS-D patients with syndrome of spleen-kidney yang deficiency is effective,and improve their intestinal flora effectively.
