摘要
目的探讨外科治疗儿童主动脉瓣病变的近中期结果及经验总结.方法收集2009年1月至2019年9月期间所收治的22例主动脉瓣病变的患儿的临床资料,男性13例,女性9例,年龄1~16岁;体重12~46 kg,平均29 kg;年龄分布:1~3岁9例,3~6岁3例,6~12岁4例,12~16岁6例.术前经超声心动图及心脏CT明确诊断,先天性主动脉瓣病变16例,后天获得性主动脉瓣病变6例.手术在全麻、体外循环下进行,结合超声心动图、心脏CTA及术中情况决定手术方式.术后3个月、6个月、1年、以后每年复查超声心动图.结果行主动脉瓣置换手术6例,主动脉瓣成形手术5例,改良Ross手术5例,Ross手术4例,Bentall术2例.术后随访:长期随访20例,2例失访,其中AVR后2例跨瓣压差>40 mm Hg,1例Ross出现中量反流,处于随访观察状态,无二次手术.结论儿童主动脉瓣病变的外科治疗方法有多样性的特点,外科治疗儿童主动脉瓣病变的近中期结果是满意的、有效的.
Objective To explore the short and medium term results and experience of surgical treatment of aortic valve disease in children.Methods Twenty-two patients with aortic valve disease were admitted between January 2009 and September 2019.There were 13 males and 9 females,aged from 1-16 years old.weight 12-46 kg,average 29 kg.Age distribution:9 cases aged from 1-3 years old,3 cases aged from 3?6 years old,4 cases aged from 6-12 years old and 6 cases aged from 12-16 years old.Sixteen cases of congenital aortic valve disease and six cases of acquired aortic valve disease were diagnosed by preoperative cardiac ultrasound and cardiac CTA.The operation was performed under general anesthesia and extracorporeal circulation and the operation mode was determined by combining cardiac ultrasound,CTA and intraoperative conditions.Echocardiography was reexamined 3 months,6 months,1 year and annually thereafter.Results Mechanical aortic valve replacement(AVR)was performed in 6 patients,aortic valve shaping(AVP)in 5 patients,modified Ross surgery in 5 patients,Ross surgery in 4 patients and Bentall surgery in 2 patients.Postoperative follow-up:20 cases were followed up for a long time and 2 cases were lost to follow-up,among which 2 cases had trans-valvular differential pressure>40 mm Hg after AVR and 1 case had medium volume regurgitation with Ross which was in a follow-up observation state;no second operation was performed.Conclusion Surgical treatment of children's aortic valve lesions has a variety of characteristics.The near and intermediate results of surgical treatment of children's aortic valve lesions are satisfactory and effective.
