摘要
目的观察益气活血方对慢性阻塞性肺疾病急性加重期(AECOPD)患者机械通气相关指标的影响。方法52例AECOPD的患者随机分为治疗组27例与对照组25例,所有患者均根据AECOPD指南接受统一治疗,治疗组在一般治疗的基础上联合益气活血方中药治疗,疗程7 d。比较两组患者有创机械通气时间、通气支持参数、拔管后序贯无创机械通气时间,住院再插管率、中医证候评分、D-二聚体、ICU住院时间、总住院时间、住院病死率、28 d病死率等。结果治疗组平均机械通气时间低于对照组(P<0.05);两组患者在通气支持参数、拔管后序贯无创机械通气时间、脱机后中医证候评分及D-二聚体等方面,治疗组明显优于对照组(P<0.05)。ICU住院时间、总住院时间、住院病死率、28 d病死率方面两组差异无统计学意义(P>0.05)。结论益气活血方治疗AECOPD患者疗效确切。
Objective:To observe the effect of Yiqi Huoxue Decoction on mechanical ventilation time in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD).Methods:52 cases of AECOPD patients were randomly divided into the treatment group(27 cases)and the control group(25 cases).All patients received unified treatment according to AECOPD guidelines.The treatment group was treated with Yiqi Huoxue Decoction on the basis of general treatment for 7 days.The following data was compared:invasive mechanical ventilation time,ventilation support parameters,sequential non-invasive mechanical ventilation time after extubation,inhospital reintubation rate,TCM syndrome score,D-dimer,ICU hospitalization time,hospitalization time,hospitalization mortality rate,28 day mortality rate,etc.Results:The average time of mechanical ventilation in the treatment group was lower than that in the control group(P<0.05).The treatment group was significantly better than the control group in ventilation support parameters,sequential non-invasive mechanical ventilation time after extubation,TCM syndrome score after offline and D-dimer(P<0.05).There was no significant difference in other observation indicators(P>0.05).Conclusion:Yiqi Huoxue Decoction can reduce the mechanical ventilation time in AECOPD patients.
