摘要
目的探讨连续性肾脏替代治疗(CRRT)中体外循环凝血的原因和预防对策。方法选取2018年8月~2019年2月我院实施CRRT的40例患者作为研究对象,给予三种不同抗凝方式治疗,观察CRRT期间体外循环凝血发生情况、凝血原因及结局。结果40例患者经CRRT治疗后,共发生体外循环凝血9例,发生率为22.50%,其中肝素治疗19例患者中3例发生凝血,低剂量肝素治疗9例患者中2例发生凝血,无肝素治疗12例患者中4例发生凝血。4例患者因管路进空气导致凝血,2例因滤器造成凝血,3例因静脉壶凝血。结论在连续性肾脏替代治疗中只有密切观察,出现凝血征兆时,查找、分析原因,进行针对性预防和处理,可降低体外循环凝血的发生,提高治疗质量。
Objective To explore the causes and preventive measures of extracorporeal blood coagulation during continuous renal replacement therapy(CRRT).Methods From August 2018 to February 2019,40 patients who underwent CRRT in our hospital were selected as the research subjects.Give three different anticoagulant treatments,observe the occurrence,causes and outcomes of coagulation during cardiopulmonary bypass during CRRT.Results After CRRT treatment,40 patients had a total of 9 cases of extracorporeal coagulation,the incidence rate was 22.50%.Among them,3 of 19 patients treated with heparin developed coagulation,and 2 of 9 patients treated with low dose heparin developed coagulation without heparin Coagulation occurred in 4 of 12 patients.4 patients had coagulation due to air in the pipeline,2 patients had coagulation due to filter,and 3 patients had coagulation due to venous ampulla.Conclusion In continuous renal replacement therapy,only close observation,when signs of coagulation appear,find and analyze the causes,and carry out targeted prevention and treatment can reduce the occurrence of extracorporeal circulation coagulation and improve the quality of treatment.
