摘要
目的:通过对生化汤产后应用古籍文献进行系统梳理,以更好地指导临床应用。方法:采用文献学的研究方法,以古籍提要类著作及妇科临床大家推荐古籍作为筛选书目标准,选取古籍15种,结合中医妇科学、西医妇产科学等多维度知识,从生化汤组方、适应证、禁忌证、调护特色以及方剂加减特色等5个方面系统总结了历代古籍中生化汤的相关记载。结果:生化汤组方特色:生化汤有行中有补,化中有生之意;主要组成药物有当归、川芎、桃仁、姜、炙甘草五味药。川芎、当归、桃仁三味善破恶血,骤生新血,取行中有补之意,配伍黑姜、甘草引三品入肺、肝,生血理气。适应证:临产状态以及产后状态,符合血瘀、寒凝证候者,均可选择生化汤治疗。禁忌证:产后生化汤需辨证运用,辨证有阴虚、热邪、暑邪、湿温以及无血瘀实证的患者慎用。调护特色:空腹热服,频用,不拘帖数服至病退方止,注意保暖以及适量粥汤善后固护脾胃之气。服药后可配合手法及外用药调护增进疗效。方剂加减特色:从产后大便燥结、寒、痰、食积等产后常见临床症状与病理状态的调治以及产后风药的运用特点梳理总结生化汤加减临床常用药物组合。结论:生化汤临床应用须继承古籍文献精华,严格辨证并结合调护原则,才能达到预期临床疗效。
Objective: To better guide clinical practice by systematically combing TCM classics of Shenghua Decoction applied in post-natal application. Methods: The research method of bibliography was used,with ancient books abstracts and gynecological clinics recommending ancient books as the selection criteria for bibliography,15 kinds of ancient books were selected,combined with multidimensional knowledge of traditional Chinese medicine and gynecology,western medicine and obstetrics and gynaecology,etc.Five aspects,including indications,contraindications,conditioning characteristics and prescription addition and subtraction characteristics,systematically summarized the relevant records of ancient biochemical soups in ancient times. Results: Features of Shenghua Decoction composition: The decoction functions in tonifying and activating Qi movement as well as production Qi and Blood.The main constituent drugs are Danggui( angelica),Chuanxiong( ligusticum wallichii),Taoren( peach kernel),Jiang( ginger),and Zhigancao( processed licorice). The former three are good at breaking blood stasis,generating new blood,meaning tonifying with cativating Qi movement. When they are compatible with Heijiang( black ginger) and Gancao( licorice),the three can be led into the l Lung and Liver,to procuce Blood and Qi. Indications: In labor and postpartum conditions,those who meet the symptoms of blood stasis and cold coagulation can choose Shenghua Decoction. Contraindications: The post-natal biochemical soup needs to be used in a dialectical manner. Patients with Yin deficiency,Heat Evil,Shu Evil,or Damp Evil,yet without Blood stasis syndrome should be used with great caution. Nursing features: drinking hot,frequent use,with as much as the decoction until the illness retreats;paying attention to keep warm and moderate porridge to protect the Spleen and Stomach Qi.
