摘要
目的探讨参麦注射液联合左卡尼汀对缺血性心肌病心力衰竭患者高敏肌钙蛋白T(hs-cTnT)、N端前脑钠肽(NT-proBNP)、超敏C反应蛋白(hs-CRP)及心功能的影响,并使用超声检查评价疗效。方法80例缺血性心肌病心力衰竭患者依据临床治疗方案不同分为观察组及对照组各40例,对照组给予吸氧、降血脂、利尿剂、抗血小板凝集、地高辛、洋地黄等治疗,观察组在对照组治疗方案基础上应用左卡尼汀、参麦注射液进行治疗,连续治疗2周。治疗前后分别测定血清hs-cTnT、NT-proBNP、hs-CRP,采用超声心动图测定LVEDV、LVESV、LVEF、LVESD、LVEDD。结果疗程结束后观察组hs-cTnT、NT-proBNP、hs-CRP低于同期对照组(P<0.05);LVEDV、LVESV、LVEDD、LVESD低于同期对照组,LVEF高于期对照组(P<0.05)。结论参麦注射液联合左卡尼汀能够有效调节神经内分泌系统,抑制炎症反应,增加心肌血氧供应,有效改善缺血性心肌病心力衰竭患者左室重构及心功能。
Objective To explore the effect of Shenmai injection combined with levocarnitine on hs-cTnT,NT-proBNP,hs-CRP,and caridac function,and evaluate the efficacy by ultrasound.Methods A total of 80 inpatients with heart failure caused by ischemic cardiomyopathy in our hospital from August,2017 to August,2018 were divided into observation group and control group with 40 cases in each group according to different treatment protocols.The control group were given oxygen inhalation,lipid decreasing,diuretics,antiplatelet aggregation,digoxin,and digitalis.On the above basis,the observation group were treated with levocarnitine and Shenmai injection.The two groups were continuously treated for 2 weeks.The serum hs-cTnT,NT-proBNP,and hs-CRP were detected before and after treatment.The echocardiogram was used to detect LVEDV,LVESV,LVEF,LVESD,and LVEDD.Results The levels of hs-cTnT,NT-proBNP,and hs-CRP after treatment in the observation group were significantly lower than those in the control group(P<0.05).LVEDV,LVESV,LVESD,and LVEDD after treatment in the observation group were significantly lower than those in the control group,while LVEF was significantly higher than that in the control group(P<0.05).Conclusions Shenmai injection combined with levocarnitine can effectively regulate the neuroendocrine system,inhibit the inflammatory reaction,and increase the cardiac blood oxygen supply,thus effectively improving the left ventricular resconsitution and cardiac function.
