摘要
目的探讨期待治疗在不同孕周未足月胎膜早破(PPROM)患者中的有效性和安全性。方法选择2016年1月至2018年12月收治的150例不同孕周PPROM患者为研究对象,其中,孕24~27^+6周患者为研究1组(n=50),孕28~33^+6周患者为研究2组(n=50),孕34~36^+6周患者为研究3组(n=50)。三组均给予期待治疗,比较三组的不良妊娠结局及新生儿并发症发生情况。结果治疗后,研究1组患者的不良妊娠结局总发生率和新生儿并发症总发生率高于研究2组及研究3组(P<0.05);研究2组与研究3组患者的不良妊娠结局总发生率和新生儿并发症总发生率无明显差异(P>0.05)。结论不同孕周PPROM患者实施期待治疗的效果不同,其对于<28周的患者应用效果不甚理想,不建议继续实施保胎治疗,对于28~33^+6周的患者进行期待治疗效果较为明显,可以明显改善患者的不良妊娠结局,降低新生儿并发症的发生率。
Objective To investigate the effectiveness and safety of expectation treatment in patients with preterm premature rupture of membranes(PPROM)at different gestational weeks.Methods From January 2016 to December 2018,150 patients with PPROM at different gestational weeks were selected as the study objects.Among them,patients at 24-27^+6 weeks of gestation were enrolled in study 1 group(n=50),patients at 28-33^+6 weeks of gestation were enrolled in study 2 groups(n=50),and patients at 34-36^+6 weeks of gestation were enrolled in study 3 groups(n=50).The three groups were given expectation treatment.The adverse pregnancy outcomes and neonatal complications in the three groups were compared.Results After treatment,the total incidence of adverse pregnancy outcomes and the total incidence of neonatal complications in the study 1 group were higher than those in the study 2 group and the study 3 group(P<0.05);there were no significant differences in the total incidences of adverse pregnancy outcomes and the total incidence of neonatal complications between the study group 2 and the study group 3(P>0.05).Conclusion The expectation treatment has different effect for the patients with PPROM at different gestational weeks.The expectation treatment for the patients less than 28 weeks of gestation does not have very idea effect,so it is not recommended to continue to carry out fetal protection treatment.The expectation treatment for the patients with 28-33^+6 weeks of gestation has more obvious effect,which can significantly improve the adverse pregnancy outcome of the patients and reduce the incidence of neonatal complications.
