定向球囊PKP治疗骨质疏松性椎体压缩骨折的临床效果

摘要 目的探讨定向球囊经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折的临床效果。方法选择2016年6月至2018年12月我院收治的68例骨质疏松性椎体压缩骨折患者为研究对象,采用Excel随机函数法将患者分为对照组和试验组,每组34例。对照组患者采用常规PKP治疗,试验组患者采用定向球囊PKP治疗。比较两组患者的椎体改善情况、功能障碍及骨水泥渗漏率。结果治疗后3个月,试验组患者的椎体前缘、椎体中线及椎体后缘高度均高于对照组及治疗前,差异具有统计学意义(P<0.05);治疗后1周、3个月,两组患者的ODI评分均低于治疗前,且试验组均明显低于对照组,差异具有统计学意义(P<0.05);治疗后,试验组患者的骨水泥渗漏总发生率为2.94%,明显低于对照组的17.65%,差异具有统计学意义(P<0.05)。结论定向球囊PKP应用于骨质疏松性椎体压缩骨折的治疗中可以明显改善患者椎体功能,缓解其功能障碍,降低骨水泥渗漏率,值得临床广泛推广应用。 Objective To investigate the clinical effect of directional balloon percutaneous kyphoplasty(PKP)in the treatment of osteoporotic vertebral compression fractures.Methods From June 2016 to December 2018,68 patients with osteoporotic vertebral compression fractures were enrolled as study objects.The patients were divided into control group and experimental group by Excel random function method,with 34 cases in each group.Patients in the control group were treated with conventional PKP,while patients in the experimental group were treated with directional balloon PKP.The vertebral body improvement,dysfunction and leakage rate of bone cement were compared between the two groups.Results At three months after treatment,the height of the anterior,middle and posterior edge of the vertebral body in the experimental group were higher than those in the control group and before treatment,the differences were statistically significant(P<0.05).At one week and three months after treatment,the ODI scores in the two groups were lower than those before treatment,and those in the experimental group were lower than the control group,the differences were statistically significant(P<0.05).After treatment,the total incidence of cement leakage in the experimental group was 2.94%,which was significantly lower than 17.65%in the control group,the difference was statistically significant(P<0.05).Conclusion The directional balloon PKP applied in the treatment of osteoporotic vertebral compression fractures can significantly improve the vertebral function,alleviate the dysfunction,and reduce the leakage rate of bone cement,which is worthy of wide clinical promotion and application.
