摘要
[目的]探讨抚触联合游泳在新生儿胆红素脑病护理中的应用效果。[方法]选取2018年1月-2018年12月新生儿科收治的86例胆红素脑病患儿,按随机数字表法分为对照组(n=43)和研究组(n=43)。两组患儿均接受基础治疗,对照组给予常规护理,研究组在常规护理基础上给予游泳联合抚触。比较两组护理干预效果。[结果]研究组患儿黄疸持续时间、黄疸消失时间、总光疗时间、胎粪初次排出时间分别为(2.62±0.72)d、(4.61±1.04)d、(46.22±3.00)h、(12.49±3.60)h,对照组黄疸持续时间、黄疸消失时间、总光疗时间、胎粪初次排出时间分别为(3.99±1.13)d、(7.88±1.11)d、(60.08±5.62)h、(17.96±5.91)h,两组比较差异均有统计学意义(P<0.001);两组患儿头围、身长、体重及上臂围增长情况比较差异有统计学意义(P<0.001);两组干预后总胆红素(TBIL)、结合胆红素(DBIL)、非结合胆红素(IBIL)、血清神经元特异性烯醇化酶(NSE)、S100蛋白和胆红素致神经功能障碍(BIND)等指标改善程度优于干预前(P<0.01或P<0.001);研究组患儿干预后TBIL、DBIL、IBIL、NSE、S100蛋白和BIND等指标均低于对照组(P<0.05)。[结论]抚触联合游泳在新生儿胆红素脑病护理中的应用有助于降低胆红素水平,遏制胆红素脑病不良进程。
Objective:To probe into the application effect of touching combined swimming in the nursing of neonatal bilirubin encephalopathy.Methods:A total of 86 cases of bilirubin encephalopathy were selected,who were admitted in neonatal department in our hospital from January 2018 to December 2018.According to random number table method,they were divided into control group and experimental group,with 48 cases in each group.Patients in two groups were given basic treatment.Patients in control group,were given routine nursing.While,patients in experimental group were given swimming combined with touching,on the basis of routine nursing.The effect of nursing intervention were compared between two groups.Results:The duration of jaundice,the receding time of jaundice,the total time of receving phototherapy,and the time of first meconium excretion of patients in experimental group were 2.62±0.72 days,4.61±1.04 days,46.22±3.00 hours,12.49±3.60 hours respectively,However,above time of patients incontrol group were 3.99±1.13 days,7.88±1.11 days,60.08±5.62 hours,17.96±5.91 hours respectively.Differences between two groups were statistically significant(P<0.001).There were significant differences in head circumference,body length,boby weight and upper arm circumference between two groups(P<0.001).The improvement of total bilirubin(TBIL),direct bilirubin(DBIL),indirect bilirubin(IBIL),serum neuron-specific enolase(NSE),S100 protein and bilirubine-induced neurological dysfunction(BIND)of patients in two groups after intervention were better than those of patients before intervention(P<0.01 or P<0.001).After intervention,TBIL,DBIL,IBIL,NSE,S100 protein and BIND in the experimental group were lower than those in the control group(P<0.05).Conclusion:The application of touching combined with swimming in the nursing of neonatal bilirubin encephalopathy could effectively reduce bilirubin levels,and curb the adverse process of bilirubin encephalopathy.
