摘要
目的探讨鼻内镜下逆行泪道支架置入手术对鼻泪管阻塞患者的治疗效果。方法将94例慢性泪囊炎鼻泪管阻塞患者纳入研究,采用随机数字表法将患者分为研究组和对照组,每组47例。研究组实施鼻内镜下逆行泪道支架置入手术治疗,对照组实施单纯泪道探通手术治疗。比较2组总有效率、泪道冲洗通畅时间、溢泪症状消除时间、术后并发症发生率以及疾病复发率。结果研究组总有效率明显高于对照组(93.62%vs.82.98%,P<0.05)。研究组泪道冲洗通畅时间和溢泪症状消除时间均短于对照组(P<0.05)。2组间并发症发生率差异无统计学意义(P>0.05)。研究组术后1年疾病复发率明显低于对照组(4.26%vs.17.02%,P<0.05)。结论与单纯泪道探通手术治疗相比,鼻内镜下逆行泪道支架置入手术治疗鼻泪管阻塞可以提高疗效,缩短泪道冲洗通畅时间和溢泪症状消除时间,降低术后复发率,且不会增加术后并发症,是一种安全、有效的治疗鼻泪管阻塞的方法。
Objective To investigate the therapeutic effect of retrograde lacrimal stent placement in patients with nasolacrimal duct obstruction under nasal endoscopy.Methods A total of 94 patients with nasolacrimal duct obstruction were enrolled in the study.The patients were divided into a study group and a control group with 47 cases in each group by random number table method.The study group was treated with retrograde lacrimal stent placement under nasal endoscope,while the control group was treated with simple lacrimal probing.The total effective rate,time of unobstructed lacrimal irrigation,elimination time of epiphora symptoms,incidence of complications and recurrence rate were compared between the two groups.Results The total effective rate of the study group was significantly higher than that of the control group(93.62%vs.82.98%,P<0.05).The time of unobstructed lacrimal irrigation and relief of epiphora symptoms in the study group were shorter than that in the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).The recurrence rate of disease in the study group was significantly lower than that in the control group(4.26%vs.17.02%,P<0.05).Conclusions Compared with the simple lacrimal passage probing operation,endoscopic retrograde lacrimal stent placement can improve the therapeutic effect,shorten the time of unobstructed lacrimal irrigation,relieve the symptoms of epiphora,reduce the recurrence rate,while do not increase the complications.It is a safe and effective treatment for nasolacrimal duct obstruction.
