摘要
目的探讨CD47阳性的白血病干细胞在急性髓细胞白血病中的表达。方法随机选取急性髓细胞白血病患者294例作为研究组,另随机选取同期健康人员38例为健康组。统计分析研究组不同FAB分型患者白血病干细胞中CD47表达情况、2组人员不同细胞亚群上CD47表达情况、研究组患者白血病干细胞中CD47表达和临床特征的相关性、研究组不同CD47表达患者的复发、死亡、平均生存时间。结果研究组294例患者中,白血病干细胞中CD47表达阳性224例,阳性表达率为76.2%。研究组患者白血病干细胞、CD34+CD38+上CD47 MFI、阳性表达率均显著高于健康组(P<0.05),白血病干细胞上CD47 MFI、阳性表达率均显著高于CD34+CD38+(P<0.05)。研究组有肝脾淋巴结大患者白血病干细胞中CD47阳性表达率显著高于无肝脾淋巴结大患者(P<0.05),WBC≥20×109/l患者白血病干细胞中CD47阳性表达率显著高于<20×109/l患者(P<0.05)。研究组CD47阳性表达患者的复发率、死亡率[46.9%(105/224)、43.8%(98/224)]均显著高于CD47阴性表达患者[24.3%(17/70)、18.6%(13/70)](P<0.05),平均生存时间显著短于CD47阴性表达患者(P<0.05)。结论急性髓细胞白血病白血病干细胞中CD47阳性表达率高。
Objective To explore the expression of CD47 positive leukemia stem cells in acute myeloid leukemia.Methods 294 patients with acute myeloid leukemia were randomly selected,who were divided into the the study group,in addition,38 healthy patients were randomly selected as healthy group.CD47 expression in leukemia stem cells of patients with different FAB types in the study group,CD47 expression in different cell subsets in the 2 groups,correlation between CD47 expression and clinical features in leukemia stem cells in the study group,Recurrence and death and mean survival time of patients with different CD47 expression in the study group were statistically analyzed.Results Among the 294 patients in the study group,224 patients showed positive CD47 expression in leukemia stem cells,the positive expression rate was 76.2%.The expression percentage of leukemia stem cells,CD34+CD38+CD47 MFI in the study group was significantly higher than that in the healthy group(P<0.05),in leukemia stem cells,The expression percentage of CD47 MFI and CD38+was significantly higher than that of CD34+CD38+(P<0.05).In the study group,the positive rate of CD47 expression in leukemia stem cells of patients with large hepatic and splenic lymph nodes was significantly higher than that of patients without large hepatic and splenic lymph nodes(P<0.05).In the case,the recurrence rate and mortality rate of patients with positive CD47 expression group were 46.9%(105/224)and 43.8%(98/224),which were significantly higher than those of patients with negative CD47 expression 24.3%(17/70)and 18.6%(13/70)(P<0.05),the mean survival time of patients with positive CD47 expression was significantly shorter than that of patients with negative CD47 expression(P<0.05).Conclusion The expression rate of CD47 in acute myeloid leukemia stem cells is high.
