呼出气冷凝液检测在肺癌合并慢性阻塞肺疾病患者中的应用分析

摘要 目的:探讨呼出气冷凝液生物标记物检测与肺癌术后患者肺功能及其并发症的相关性.方法:以我院2016年3月~2018年11月行肺叶切除术的72例肺癌患者作为研究对象,根据入院时的肺功能检测结果将合并有慢性阻塞性肺疾病的36例患者列为观察组,无慢性阻塞性肺疾病的36例患者列为对照组,对两组患者手术前后呼出气冷凝液中肿瘤坏死因子(TNF-α)和白介素-1β(IL-1β)水平(ELISA法)、肺功能及并发症的发生情况进行比较,并对呼出气冷凝液中肿瘤坏死因子(TNF-α)和白介素-1β(IL-1β)水平与肺功能、并发症的相关性进行分析.结果:两组患者术后1d其EBC中TNF-α和IL-1β水平均较术前1d明显增高,而术后7d后明显降低(P<0.05);组间比较,观察组术前1d、术后1d以及术后7d的TNF-α和IL-1β水平均明显高于对照组(P<0.05);观察组肺组织TNF-α和IL-1β含量明显高于对照组(P<0.05);术前1d观察组FEV1、FEV1/FVC和FEV1%水平均明显低于对照组(P<0.05);术后7d观察组共发生并发症13例,对照组共发生并发症5例,组间比较观察组术后并发症发生率明显高于对照组(P<0.05);经Pearson相关分析显示,肺组织中TNF-α含量与EBC中TNF-α水平呈正相关(r=0.434,P<0.05);肺组织中IL-1β含量与EBC中IL-1β水平呈正相关(r-=0.508,P<0.05).EBC中TNF-α水平与FEV1%呈负相关(r=-0.422,P<0.05);EBC中IL-1β水平与FEV1%呈负相关(r=-0.404,P<0.05).EBC中TNF-α和IL-1β水平与FEV1、FEV1/FVC以及并发症发生率并无相关性(P>0.05).结论:呼出气冷凝液中TNF-α和IL-1β与肺癌患者肺组织中TNF-α和IL-1β水平呈正相关,可反映患者的肺功能水平,对临床诊疗具有一定的价值. Objective:To investigate the correlation between exhaled condensate biomarker detection and lung function and complications in patients with lung cancer.Methods:72 patients with lung cancer who underwent lobectomy from March 2016 to November 2018 in our hospitial were included in the study.According to the results of pulmonary fiinction tests at admission,36 patients with chronic obstructive pulmonary disease were included in the observation group,and 36 patients without chronic obstructive pulmonary disease were included in the control group.The levels of TNF-α and IL-10(ELISA),pulmonary fiinction and complications in exhaled breath condensate before and after surgery in both groups were compared.The correlation between TNF-α and IL-1β levels in exhaled breath condensate and lung function and complications was analyzed.Results:At 1 day after operation,the levels of TNF-α and IL-10 in EBC were significantly higher than those before operation,and decreased significantly after 7 days(P<0.05).The levels of TNF-α and IL-1 p in the observation group were significantly higher than those in the control group at 1 day before surgery,1 day and 7 days after operation(P<0.O5).The levels of TNF-α and IL-1 β in the lung tissue of the observation group were significantly higher than those in the control group(P<0.05).At 1 day before operation,the FEV1,FEV1/FVC and FEV1%levels in the observation group were significantly lower than those in the control group(P<0.05).At 7 days after operation,13 patients had complications in the observation group and 5 patients had complications in the control group.The incidence of postoperative complications was significantly higher in the observation group than in the control group(P<0.05).Pearson correlation analysis showed that TNF-α content in lung tissue was positively correlated with TNF-α level in EBC(r=0.434,P<0.05).There was a positive correlation between IL-1 B content in lung tissue and IL-1β level in EBC(P=0.508,P<0.05).The level of TNF-α in EBC was negatively correlated
