摘要
目的探讨在完全腹腔镜右半结肠癌切除术中应用重叠式三角吻合法进行消化道重建的安全性及可行性。方法选取2017年1月至2019年3月首都医科大学大兴教学医院接受微创手术的右半结肠癌患者82例,按照随机数表法将患者分为对照组(41例,接受改良三角吻合法)和观察组(41例,接受重叠式三角吻合法),比较两组患者的吻合时间、术中出血量和平均手术时间,术后疼痛程度[视觉模拟评分(visual analogue score,VAS)]、首次排气时间、首次进食时间以及术后并发症的发生率。结果两组患者的吻合时间、平均手术时间、首次排气时间、VAS比较差异有统计学意义(P<0.05);平均出血量、首次进食时间以及并发症的发生率比较差异无统计学意义(P>0.05)。结论重叠式三角吻合法与传统改良三角吻合法应用于完全腹腔镜右半结肠癌切除术的消化道重建效果均良好,且重叠式三角吻合法吻合时间和手术时间较少,值得临床推广。
Objective To explore the safety and feasibility of overlapping triangular anastomosis in laparoscopic right colon cancer resection.Methods A total of 82 patients with right colon cancer who underwent minimally invasive surgery in Daxing Teaching Hospital,Capital Medical University from January 2017 to March 2019 were selected as subjects.The patients were divided into two groups according to the random number table method,41 cases were in each group.Overlapping triangular anastomosis was used in the observation group,the traditional modified triangular anastomosis was used in the control group.The anastomosis duraion,intraoperative blood loss and mean operative duration,postoperative visual analogue score(VAS),the first time of intestinal exhaust and feeding,and postoperative complications were compared between the two groups.Results There were significant differences in anastomosis duration,mean operative duration,the first time of intestinal exhaust and VAS between the two groups(P<0.05).There were no significant differences in the mean blood loss,the first feeding time,and the incidence of complications(P>0.05).Conclusions The overlapping triangular anastomosis used in the laparoscopic right colon cancer resection has good effect of gastrointestinal reconstruction as the traditional modified triangular anastomosis,but the duration of anastomosis and operation is much shorter.It is worthy of clinical application.
