摘要
目的:探讨基于自评-主观全面评定(PG-SGA)量表的营养干预在膀胱癌行根治性全膀胱切除术患者围术期中的应用效果。方法:选择2018年1月1日~2019年12月31日收治的104例肌层浸润性膀胱癌行根治性膀胱切除术患者作为研究对象,将2018年1月1日~2019年1月31日的52例分为对照组,采用常规营养干预;将2019年2月1日~12月31日的52例分为实验组,在常规营养干预基础上采用基于PG-SGA的营养干预。统计两组干预后第1、2、3周的营养摄入量(包括能量及蛋白质),同时比较两组干预后第3周的营养指标[包括转铁蛋白(TRF)、血清白蛋白(ALB)、血清前白蛋白(PAB)],并评价术后生活质量及护理质量满意度。结果:实验组营养干预后第1、2、3周的营养摄入中能量、蛋白质量均高于对照组同期(P<0.01),并且营养摄入中能量、蛋白质量随干预后时间的增加而不断增加,营养干预3周后实验组TRF、ALB、PAB水平均高于对照组(P<0.01);营养干预后第3周,实验组食欲、睡眠、精神、日常活动、与人交往评价得分均高于对照组(P<0.01);实验组营养干预质量满意率高于对照组(P<0.05)。结论:PG-SGA量表下的营养干预对改善肌层浸润性膀胱癌行根治性全膀胱切除术患者围术期的营养状况效果满意,可有效提高其营养摄入量,改善营养指标,提高术后生活质量,护理质量满意度高。
Objective:To investigate the effect of nutritional intervention based on the PG-SGA scale in the perioperative period of radical cystectomy for bladder cancer.Methods:A total of 104 cases of radical cystectomy for muscle-invasive bladder cancer admitted from January 1,2018 to December 31,2019 were selected as the study subjects.Fifty-two patients from January 1,2018 to January 31,2019 were divided into the control group,and the routine nutritional interventions were implemented.Fifty-two patients from February 1 to December 31,2019 were divided into the experimental group and nutritional intervention based on PG-SGA was used on the basis of routine nutritional intervention.The nutritional intake(including energy and protein)at the first,second and third week after the intervention of the two groups was calculated,the nutritional indicators(including TRF,ALB and PAB)at the third week after the intervention of the two groups were compared,and postoperative life quality and nursing quality satisfaction were evaluated.Results:The energy and protein in nutrition intake of the experimental group were higher than that of the control group at the first,second and third week after nutritional intervention(P<0.01),and the energy and protein in nutrition intake increased with the time after intervention.The levels of TRF,ALB and PAB in the experimental group were higher than that of the control group at the third week after nutritional intervention(P<0.01).At the 3rd week after nutritional intervention,the scores of appetite,sleep,spirit,daily activities and interpersonal communication in the experimental group were all higher than those in the control group(P<0.01).The satisfaction rate of nutritional intervention in the experimental group was higher than that in the control group(P<0.05).Conclusion:Nutritional intervention under the PG-SGA scale can improve the perioperative nutritional status of patients undergoing radical cystectomy for muscular invasive bladder cancer,effectively increase their nutritional intake,impro
