脑氧饱和度监测在临床麻醉中的应用

摘要 目的探讨脑氧饱和度监测在临床麻醉中的应用。方法将2017年2月~2019年2月我院收治的100例临床患者(需麻醉)作为研究对象,采用随机数字表法分为对照组(无脑氧饱和度监测)和观察组(脑氧饱和度监测),每组患者50例。比较PETCO2(呼末二氧化碳分压)变化对rSO2(脑组织氧饱和度)的影响及手术过程中rSO2与PETCO2之间的变化。结果PETCO2在正常范围值内,rSO2与PETCO2有一定的相关性,即rSO2随着PETCO2的升高其数值也相应升高,且PETCO2指标达到40mm Hg后,每升高5mm Hg后,rSO2数值显著升高;差异均有统计学意义(P<0.05)。经脑氧饱和度监测的组别出现昏迷、神经损伤的总并发症发生率显著低于对照组的总并发症发生率,且观察组的复发率显著低于对照组复发率,观察组的住院费用也显著低于对照组住院费用,差异有统计学意义(P<0.05)。结论对于临床需要麻醉手术的患者,采用脑氧饱和度监测,有利于麻醉的调整,减少围术期并发症的发生率,降低复发率的同时减少住院花费的医疗费用,有较高临床应用价值。 Objective To explore the application of cerebral oxygenation monitoring in clinical anesthesia.Methods A total of 100 patients(anesthesia required)admitted in our hospital from February 2017 to February 2019 were selected as the research subjects.They were divided into control group(without cerebral oxygenation monitoring)and observation group(with cerebral oxygenation monitoring)using random number table method,with 50 cases in each group.The effects of changes in end tidal CO2 tension(PETCO2)on regional oxygen saturation(rSO2)and the correlation between rSO2 and PETCO2 were explored.Results In the normal range of PETCO2,rSO2 had a certain correlation with PETCO2,that is,rSO2 increased with the increase of PETCO2,and after the PETCO2 index reached 40mm Hg,rSO2 value increased significantly with each increase of 5mmHg,and the differences were statistically significant(P<0.05).The total complications rate of coma and nerve injury,recurrence rate and hospitalization cost in the observation group was significantly lower than that in the control group,and the differences were statistically significant(P<0.05).Conclusion For patients who need anesthesia in clinical practice,the use of cerebral oxygenation monitoring is conducive to the adjustment of anesthesia,reduces the incidence of perioperative complications,recurrence rate,and medical costs of hospitalization,which has high clinical application value.
