摘要
目的了解我院近4年化疗药物性肝损伤病例的发生情况及规律,为临床合理用药提供参考。方法回顾性分析2015年7月1日~2018年12月31日于我院就诊的76例化疗药物性肝损伤患者的各项资料,如性别、年龄、引起肝损伤化疗药物、发生肝损伤时间及严重程度、保肝药物的治疗效果、保肝药物的经济性情况等,进行统计分析。结果76例化疗药物肝损伤患者中男40例,女36例;肝损伤患者中多种化疗药物综合作用占61.00%;化疗药物用药1~2周内出现肝损伤占76.39%,且以引起轻度肝损伤为主,占93.42%;临床医生主要使用异甘草酸镁注射液、注射用还原谷胱甘肽、复方甘草酸苷注射液、舒肝宁注射液4种保肝药物中的1种进行治疗,经过效果成本分析,舒肝宁注射液无明显的治疗优势。结论化疗药物性肝损伤临床表现多样,发生原因复杂。医药工作者应积极开展预防和控制工作,以更好地保障患者用药安全。
Objective To understand the occurrence and regularity of chemotherapeutic drug-induced liver injury in our hospital in the past 4 years,and to provide reference for clinical rational drug use.Methods A retrospective analysis method was adopted to perform statistical analysis on the characteristics of 76 patients with chemotherapeutic drug-induced liver injury who were admitted to our hospital from July 1,2015 to December 31,2018,such as gender,age,chemotherapy drugs that cause liver damage,the time and severity of liver injury,the therapeutic effect of liver-protecting drugs,and the economics of liver-protecting drugs.Results Among the 76 patients with chemotherapeutic drug-induced liver injury,40 were male and 36 were female;61.00%of the liver injury was caused by multiple chemotherapy drugs,76.39%occurred within 1-2 weeks after chemotherapy,and 93.42%was mild liver injury.The clinicians mainly used one of the following four liver-protecting drugs for treatment:magnesium isoglycyrrhizinate injection,reduced glutathione for injection,compound glycyrrhizin injection,and Shuganning injection.According to the effect cost analysis,Shuganning injection had no obvious therapeutic advantage.Conclusion The clinical manifestations of chemotherapeutic drug-induced liver injury are diverse and the causes are complicated.Medical workers should actively carry out prevention and control work to better protect patients'medication safety.
