摘要
目的研究单采血小板中乳酸脱氢酶(LDH)水平与患者输注血小板(PLT)后PLT参数的相关性,寻找评价血小板输注疗效的指标。方法利用Trima全自动血细胞单采机完成PLT采集,留取样本5 mL,在22℃温度条件下,不同保存时间(第1、3、5天)观察PLT数量和LDH水平的变化,采用重复测量的方差分析比较22℃条件下,不同时间(第1、3、5天)的PLT、LDH差异。通过SHINOW 9.0现代血站管理信息系统追踪单采PLT临床使用后的情况,采用spearman相关分析进行采输PLT中LDH差值与输注前后PLT相关参数(MPV、PCT、PDW)差值的相关性分析。结果随贮存时间延长,在22℃条件下,PLT数量没有明显变化,LDH水平呈逐渐升高(P<0.05)。采输LDH差值与患者输注前后PCT差值相关系数是0.476,P值是0.005,相关性具有统计学意义。结论从血小板本身质量,采输PLT中LDH差值可以作为评价PLT输注疗效的指标。
Objective To study the correlation between lactate dehydrogenase(LDH)levels in apheresis platelets and PLT parameters after transfusion of platelets in patients,and to find indicators for evaluating the efficacy of platelet transfusion.Methods PLT was collected by Trima automatic blood cell single-mining machine.The sample was taken 5 mL.The changes of PLT quantity and LDH level were observed under different storage time(1,3,5 days)under the temperature of 22℃.The variance of repeated measurement was used.The differences in PLT and LDH at different times(days 1,3,and 5)at 22℃were analyzed and compared.The SHINOW 9.0 Modern Blood Station Management Information System was used to track the clinical use of APL.Spearman correlation analysis was used to analyze the correlation between the difference of LDH in PLT and the PLT related parameters(MPV,PCT,PDW)before and after infusion.Results With the prolongation of storage time,the number of PLT did not change significantly at 22℃,and the level of LDH increased gradually(P<0.05).The correlation coefficient between the LDH difference and the PCT difference before and after the patient’s infusion was 0.476,and the P value was 0.005.The correlation was statistically significant.Conclusion From the quality of the platelets themselves,the difference in LDH in the PLT can be used as an indicator to evaluate the efficacy of PLT infusion.
