摘要
目的探讨骨盆囊型包虫病X线、CT、MRI影像学征象特征。方法回顾性分析15例骨盆囊型包虫病患者影像学资料,其中4例行X线平片,15例行CT检查,5例行MRI检查,8例行CT或MRI增强检查。结果病变发生在单侧13例,双侧2例,共累及38个部位;X线表现为单一囊状膨胀性骨质破坏2例,不规则骨质破坏1例,囊状膨胀性骨质破坏合并斑片状骨质破坏1例;侵入周围软组织4例,发生钙化2例,骶髂关节变窄3例;CT表现为囊状膨胀性骨质破坏9例,溶骨性骨质破坏3例,虫蚀状骨质破坏3例,囊状膨胀性、溶骨性骨质破坏同时出现1例;侵入周围软组织13例,单囊型病灶10例、多子囊型4例,发生钙化9例;MRI表现为单囊型3例,呈长T1混杂或不混杂长T2信号,压脂序列呈高信号;多子囊型2例;囊壁弧形钙化1例,呈长T1短T2信号;继发性感染1例,脂肪间隙明显不均匀强化。结论骨盆囊型包虫病变多呈囊状膨胀性、溶骨性骨质破坏,易侵犯周围软组织,常伴有钙化,多子囊型囊肿具有特征性。
Objective To investigate the X-ray, CT and MRI imaging features of cystic echinococcosis of the pelvis. Methods The imaging features of 15 patients with pelvic bone cystic echinococcosis were retrospectively analyzed, including X-ray films in 4 patients, CT scanning in 15 patients, MRI in 5 patients, and enhanced CT or MRI in 8 patients. Results The lesion was found in unilateral pelvis in 13 patients and bilateral pelvis in 2, involving in 38 sites. X-ray findings showed single cystic expansive bone destruction in 2 patients, irregular bone destruction in 1, and cystic expansive bone destruction with patchy bone destruction in 1;invasion of surrounding soft tissue in 4, calcification in 2, and sacroiliac joint narrowing in 3. CT findings showed cystic expansive bone destruction in 9 patients, lytic bone destruction in 3, insect-eroded bone destruction in 3, and both cystic expansive bone destruction and lytic bone destruction in 1;invasion of the surrounding soft tissue in 13, single cystic lesion in 10, multiple cystic lesion in 4 and calcification in 9. MRI findings showed single capsule type in 3 patients, long signal of T1, mixed or not mixed hyperintensity on T2 and hyperintensity on STIR;multiple ascomysts in 2;arc-shaped calcification in capsule wall in 1, long signal of T1 and short T2;secondary infection in 1, both obvious non-uniform enhancement in fat gas. Conclusion Pelvic bone cystic echinococcosis is characterized by cystic dilatation and lytic destruction of bone, easy to invade the surrounding soft tissue, often accompanied by calcification and polycystic cysts.
