摘要
目的探讨穿支皮瓣修复手外伤所致手部皮肤软组织缺损的临床疗效。方法选取2016年6月至2018年10月梅州市中医医院烧伤整形疮疡科收治的60例手外伤所致手部皮肤软组织缺损患者作为研究对象,并按照随机数表法将其随机分为研究组(30例)与对照组(30例),其中研究组患者采用穿支皮瓣修复术予以治疗,对照组患者采用胸腹部带蒂皮瓣修复术予以治疗,对比观察两组患者的临床疗效及并发症发生情况结果术后21 d,研究组患者治疗总有效率为93.33%,显著高于对照组患者的治疗总有效率70.00%,差异具有统计学意义(Mann-Whitney U=317.000,Z=-2.125,P=0.034);治疗过程中,研究组患者并发症发生率为10.00%,显著低于对照组患者的并发症发生率46.67%,差异具有统计学意义(χ^2=9.932,P=0.002)。结论与胸腹部带蒂皮瓣相比,穿支皮瓣修复手外伤所致手部皮肤软组织缺损的疗效和安全性更佳,值得临床推广应用。
Objective To analyze the curative effect of perforator flap in repairing hand skin and soft tissue defects caused by hand trauma.Methods Sixty patients with hand skin and soft tissue defects caused by hand trauma,admitted to Department of Burns,Wounds and Ulcers&Plastic Surgery,Meizhou Hospital of Traditional Chinese Medicine from June 2016 to October 2018 were selected as subjects and then divided,according to random number table,into a study group(30 cases)and a control group(30 cases).Wounds in the study group were repaired with perforator flaps and wounds in the control group with pedicled thoracoabdominal flaps.The clinical effect and complications of patients in the two groups were compared and observed.Results 21 days after treatment,the total effective rate in the study group was93.33%,significantly higher than 70.00% in the control group and the results of comparison showed statistically significant difference(Mann-Whitney U=317.000,Z=-2.125,P=0.034;During treatment,the incidence of complications in the study group was 10.00%,significantly lower than 46.67% in the control group and the results of comparison showed statistically significant difference(χ^2=9.932,P=0.002).Conclusion Compared with pedicled thoracoabdominal flap,perforator flap is safer and has better curative effect in repairing hand skin and soft tissue defects caused by hand trauma,so it deserves to be promoted in clinical practice.
