摘要
坏死性筋膜炎合并糖尿病的发病率呈逐渐上升趋势,然而在临床中并不常见,极易误诊,造成严重后果,致残、致死率较高。早期诊断、积极引流,有效抗生素的应用是治疗的关键。由于糖尿病的存在可致局部组织缺血,若采用激进的清创可使清创区域的组织渐进性坏死,区域逐渐扩大,不利于创面修复。江西省中西医结合医院普外二科收治1例急性坏死性筋膜炎合并糖尿病患者,通过胆道镜微创治疗取得满意效果。
The incidence of necrotizing fasciitis(NF)complicated with diabetes mellitus is gradually increasing.However,it is not common in clinical practice,and it is easy to be misdiagnosed and cause serious consequences,with a high disability and mortality.Early diagnosis,positive drainage and the application of effective antibiotics are the key points of treatment.Since the presence of diabetes can lead to local tissue ischemia,radical debridement can lead to progressive necrosis of the tissue in the debridement area and gradual expansion of the area,which is not conducive to wound repair.A patient with acute NF complicated with diabetes was admitted to the Second Department of General Surgery of Jiangxi Hospital of Integrated Traditional Chinese and Western Medicine,and satisfactory results were achieved by minimally invasive choledochoscopy treatment.
