摘要
目的探讨腹腔镜卵巢畸胎瘤切除术中瘤体破裂的影响因素及预后效果。方法选取2016年1月至2018年12月在南通市通州区人民医院行腹腔镜卵巢畸胎瘤切除术的52例患者进行分组研究,根据瘤体是否破裂分为两组。甲组21例,共22个瘤体破裂;乙组31例,未破裂瘤体35个。比较两组患者肿瘤情况、手术情况及预后情况。结果两组肿瘤位置、大小比较,差异无统计学意义(P>0.05),但甲组周围粘连比率显著高于乙组,手术时间显著长于乙组(P<0.05)。两组术中出血量及预后情况比较,差异无统计学意义(P>0.05)。结论在腹腔镜卵巢畸胎瘤切除术中,周围粘连、手术时间长是导致瘤体破裂的主要原因,瘤体破裂后给予恰当处理后,基本不会影响患者预后。
Objective To investigate the influencing factors and prognosis of tumor rupture during laparoscopic resection of ovarian teratoma.Methods A total of 52 patients who underwent laparoscopic resection of ovarian teratoma in Nantong Tongzhou People's Hospital from January 2016 to December 2018 were divided into two groups according to whether the tumor was ruptured.There were 22 tumors ruptured in group A(21 cases),35 tumors did not rupture in group B(31 cases).The tumor status,operation status and prognosis of the two groups were compared.Results There was no significant difference in tumor location and size between the two groups(P>0.05),but the adhesion rate in group A was significantly higher than that in group B(P<0.05),and the operative time was significantly longer than that of group B(P<0.05).There was no significant difference in intraoperative blood loss and prognosis between the two groups(P>0.05).Conclusion In laparoscopic teratoma of ovary resection,surrounding adhesion and long operation time are the main causes of tumor rupture.After tumor rupture is properly treated,the prognosis of patients will not be affected.
