摘要
目的探讨女性高血压患者阻塞性睡眠呼吸暂停低通气综合征(OSAHS)的影响因素,并构建筛查模型。方法选取2016年3月至2018年8月在本院进行治疗的高血压女性患者185例,进行睡眠监测,根据监测结果分为OSAHS组(n=89)和非OSAHS组(n=96)。采集患者的基本信息,并检测患者的血压和生化指标。通过组间比较初筛OSAHS的相关因素,构建Logistic回归模型,同时评估模型的准确度、灵敏度、特异度和F1分数。结果两组患者体重指数(BMI)、腰臀比、收缩压、舒张压、低密度脂蛋白胆固醇水平、C反应蛋白、氨基末端B型利钠肽前体差异具有统计学意义(P<0.05)。Logistic回归分析表明,BMI、收缩压、舒张压、低密度脂蛋白胆固醇是影响高血压患者发生OSAHS的独立危险因素,OR值(95%CI)分别为3.493(1.564~4.245)、1.458(1.322~1.567)、3.146(1.064~4.032)、2.672(1.007~2.968)。Logistic回归模型评价结果表明,预测OSAHS的AUC为0.816,模型准确性为0.778,灵敏度为0.775,特异度为0.781,F1分数为0.778。结论本研究构建了女性高血压人群OSAHS筛查模型,探讨其中的危险因素,具有一定的临床应用价值。
Objective To investigate the influencing factors of obstructive sleep apnea hypopnea syndrome(OSAHS) in hypertensive women and to construct the screening model.Methods 185 cases of hypertensive women who received treatment in our hospital from March 2016 to August 2018 were selected.According to the sleep monitoring results,they were divided into OSAHS group(n=89) and non-OSAHS group(n=96).Basic information and other biochemical indexes were collected.Influencing factors of OSAHS were selected based on comparison between groups.The accuracy,sensitivity,specificity and F1 score were calculated.Results The body mass index(BMI),waist-to-hip ratio,systolic blood pressure,diastolic blood pressure,low-density lipoprotein cholesterol,C reactive protein,N-terminal pro-B-type natriuretic peptide levels were significantly different between two groups(P<0.05).Logistic regression analysis showed that BMI,systolic blood pressure,diastolic blood pressure,and low-density lipoprotein cholesterol were independent risk factors for OSAHS in hypertensive patients.The OR(95%CI) were 3.493(1.564-4.245),1.458(1.322-1.567),3.146(1.064-4.032) and 2.672(1.007-2.968),respectively.The logistic regression model was with the AUC of 0.816,the accuracy was 0.778,the sensitivity was 0.775,the specificity was 0.781,and the F1 score was 0.778.Conclusion The study established an OSAHS screening model for female hypertensive patients,and investigated the risk factors,which have some clinical practical value.
