摘要
目的:观察维拉帕米联合强心苷类正性肌力药治疗冠状动脉粥样硬化性心脏病(冠心病)快速房颤合并急性心力衰竭的临床效果。方法:选取84例冠心病快速房颤合并急性心力衰竭患者作为研究对象,依照随机数字表法分为对照组和研究组各42例。对照组采用强心苷类正性肌力药治疗,研究组在对照组的基础上联合维拉帕米治疗,比较两组临床疗效、治疗前后收缩压(SBP)、舒张压(DBP)、心率(HR)水平及不良反应发生率。结果:研究组治疗总有效率为95.24%(40/42),明显高于对照组的76.19%(32/42),差异有统计学意义(P<0.05);治疗3 d后,研究组SBP、DBP及HR水平均明显低于对照组,差异有统计学意义(P<0.05);两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论:维拉帕米联合强心苷类正性肌力药治疗冠心病快速房颤合并急性心力衰竭效果显著,可有效降低患者血压、心率,且安全性高。
Objective:To investigate clinical effects of Verapamil combined with cardiac glycoside inotropic agents in treatment of coronary heart disease and rapid atrial fibrillation complicated with acute heart failure.Methods:84 patients with coronary heart disease and rapid atrial fibrillation complicated with acute heart failure were selected as the research objects,and were divided into control group(n=42)and study group(n=42)according to the random number table method.The control group was treated with cardiac glycoside inotropic agents,while the study group was given Verapamil on the basis of that of the control group.The incidence of adverse reactions,clinical efficacy,and systolic blood pressure(SBP),diastolic blood pressure(DBP)and heart rate(HR)levels before and after the treatment were compared between the two groups.Results:The total effective rate in the study group was 95.24%(40/42),which was significantly higher than 76.19%(32/42)in the control group,and the difference was statistically significant(P<0.05).After 3 days of treatment,the levels of SBP,DBP and HR in the study group were significantly lower than those in the control group,and the differences were statistically significant(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions:Verapamil combined with cardiac glycoside inotropic agents in the treatment of coronary heart disease and rapid atrial fibrillation complicated with acute heart failure is effective.It can reduce blood pressure and heart rate with high safety.
