摘要
目的:分析某儿童医院抗肿瘤药不良反应(ADR)的发生情况,为临床合理用药提供参考。方法:采用回顾性分析的方法,收集该院不良反应监测中心2015年1月~2019年12月的264例儿童使用抗肿瘤药的不良反应,对患儿一般状况、药品名称、给药途径、累及系统、发生时间、因果关系、对疾病的影响、ADR类型等进行分析。结果:264例ADR中,男童多于女童;ADR排名前3名的药物分别为阿糖胞苷、甲氨蝶呤、环磷酰胺;给药途径主要为静脉滴注(泵内滴注)和肌内注射;累及系统前3位为血液系统、消化系统、皮肤及其附件;ADR多发生在用药3天内,其中用药期间发生的ADR占所有ADR的26.14%;因果关系评价中判定为“肯定”者占2.33%,“很可能”占65.91%,“可能”占31.06%;导致住院延长占32.20%,49.62%未引起病情明显变化。结论:临床应重视抗肿瘤药在患儿中引起的ADR,注意监测患儿用药后严重ADR的发生,确保用药安全。
Objective:To analyze the adverse reactions(ADRs)in children treated with antitumor drugs in a children’s hospital and provide suggestions for clinical rational drug use.Methods:264 cases of ADRs related to antitumor drugs from Jan.2015 to Dec.2019 were included in the current study and analyzed through retrospective statistical analysis.Patient’s age,gender,drug categories,drug delivery approach,organs and systems involved,time when ADRs occurred,casual connection,influence on the disease,types of ADRs and other factors were analyzed respectively.Results:In 264 cases of ADRs,the probability of occurrence is higher in male children than in females.The top three antitumor drugs with higher chance of inducing ADRs were cytarabine,methotrexate,cyclophosphamide.As for the drug delivery approach,ADRs are mainly happened in intravenous infusion(intra-pump injection)and intramuscular injection.Top three systems/organs involved in ADR were hematological system,gastrointestinal system,skin and its’appendages.Most ADRs occurred within 3 days after drug administration,among which 26.14%happened during medication.Considering the causality assessment,"Positive"accounted for 2.33%,"Probable"and"Possible"accounted for 65.91%and 31.06%respectively.ADRs caused prolonged hospitalization accounted for 32.20%and 49.62%of ARDs had no significant influence on the disease.Conclusion:Clinical attention should be paid to the ADRs caused by anti-tumor drugs in children.Clinicians should pay attention to monitor severe ADRs,and ensure the safety use of anti-tumor drugs.
