不同程度认知障碍与甲状腺功能测定的相关性 被引量:5

Correlation between Different Degrees of Cognitive Impairment and Thyroid Function

摘要 目的通过不同程度的认知障碍与甲状腺功能测定的相关性研究,为认知障碍的病因诊断方面提供一些科学依据。方法选取2017年1月至2019年06月在云南省第三人民医院神经内科就诊的存在认知功能障碍患者53例,同期健康无认知障碍者44例作为对照组。收集入组者一般信息,根据病史、查体、简易智力状态检查(MMSE)量表、日常生活活动能力(ADL)量表、头颅核磁共振等将病例组进行分组,采用电化学发光法测定入组者空腹静脉血中T3、FT3、T4、FT4、TSH的浓度及Anti-TG、TPOAB水平测定,比较各组水平差异。结果各组甲状腺功能测定的比较结果:4组间FT3水平P值为0.004,其中中度认知障碍、重度认知障碍P值分别为0.001、0.005,提示中重度认知障碍与对照组之间FT3水平有统计学意义,其余T3、T4、FT4、TSH、Anti-TG、TPOAB水平均差异统计学意义(P>0.05);5组甲状腺激素水平与MMSE评分的相关性分析结果:FT3与MMSE之间存在正相关关系,T3、T4、FT4、TSH、Anti-TG、TPOAB和MMSE均不存在相关关系。结论FT3与中重度认知障碍具有相关关系,且随着认知程度的加重呈递减趋势。 Objective To provide some scientific basis for the diagnosis of cognitive impairment by studying the correlation between different degrees of cognitive impairment and thyroid function measurement.Methods Fifty-three cases of patients with cognitive impairment who were admitted to the department of neurology of the Third people's Hospital of Yunnan province from January 2017 to June 2019 were selected as the control group.We collected the general information,according to the history,physical examination,simple mental state examination(MMSE)rating scale,active ability of daily life(ADL)scale,head MRI will case group to group,such as Electrochemical luminescence method into the group T3,FT3,T4,FT4,TSH and Anti-TG,TPOAB in fasting venous blood.Results The results of comparison of thyroid function in each group showed that the value of FT3 was P=0.004,among which the P values of moderate cognitive impairment and severe cognitive impairment were 0.001 and 0.005,respectively,suggesting that the FT3 level between moderate and severe cognitive impairment and the control group was statistically significant,while the other T3,T4,FT4,TSH,Anti-TG and TPOAB levels were not statistically significant.The results of correlation analysis between thyroid hormone level and MMSE score in the 5 groups showed that there was a positive correlation between FT3 and MMSE,but no correlation between T3,T4,FT4,TSH,Anti-TG,TPOAB and MMSE.Conclusion FT3 is correlated with moderate and severe cognitive impairment,and it tends to decrease with the increase of cognitive degree.
