摘要
目的:探索超声对婴幼儿甲状腺测值对先天性甲状腺减退症预后诊断效能研究。方法:选取2015年8月至2016年8月四川省新生儿遗传代谢病筛查绵阳分中心筛查的74例先天性甲状腺功能减退症作为研究组,同期间74例健康婴幼儿作为对照组,对研究组与对照组进行回顾性分析,内容包括所有婴幼儿1月龄、6月龄、12月龄、24月龄、36月龄甲状腺左右径、前后径、上下径及甲状腺峡部(前后径)大小。结果:研究组与对照组不同月龄甲状腺前后径均低于对照组,差异具有统计学意义(P<0.05);两组不同月龄甲状腺左右径、上下径、峡部(前后径)测值无明显差异(P>0.05);研究组连续治疗至3岁甲状腺发育增大与血清学甲状腺功能正常(TSH、T3、T4)呈正相关(r=0.99,P<0.05);研究组连续治疗至3岁甲状腺发育减小与血清学甲状腺功能正常(TSH、T3、T4)呈负相关(r=-0.912,P<0.05)。结论:超声动态监测婴幼儿甲状腺测值尤其是甲状腺前后径测值对先天性甲状腺减退症的预后有提示作用。
Objective:To investigate the effect of ultrasound monitoring of thyroid measured values on evaluating the prognosis of infants with congenital hypothyroidism.Methods:A total of 74 infants with congenital hypothyroidism who were screened out by Mianyang Branch of Sichuan Provincial Center of Screening for Neonatal Inherited Metabolic Diseases from August 2015 to August 2016 were enrolled as study group,and 74 healthy infants were enrolled as control group.A retrospective analysis was performed for related data,including left-right diameter,anteroposterior diameter,and superior-inferior diameter of the thyroid and the size of the isthmus of thyroid gland(anteroposterior diameter)at the age of 1,6,12,24,and 36 months.Results:Compared with the control group,the study group had a significantly lower anteroposterior diameter of the thyroid at different ages in months(P<0.05).There were no significant differences between the two groups in the left-right diameter and the superior-inferior diameter of the thyroid and the anteroposterior diameter of the isthmus of thyroid gland at different ages in months(P>0.05).In the patients of study group,who were continuously treated until the age of 3 years,the increase in the size of the thyroid was positively correlated with normal results of the serum thyroid function test(TSH,T3,and T4)(r=0.99,P<0.05),and the decrease in the size of the thyroid was negatively correlated with normal results of the serum thyroid function test(TSH,T3,and T4)(r=-0.912,P<0.05).Conclusion:Dynamic ultrasound monitoring of thyroid measured values,especially the anteroposterior diameter of thyroid,has a certain effect in evaluating the prognosis of congenital hypothyroidism.
