摘要
目的探讨鼻中隔多形性腺瘤的临床表现,病理学特征、诊断、治疗及预后,旨在提高耳鼻咽喉科医生对该病的认识和治疗水平,并减少漏诊和误诊。方法报告1例鼻中隔多形性腺瘤误诊病例的临床资料,并复习鼻腔多表性腺瘤国内外相关文献。结果CT扫描示左侧鼻中隔近鼻前庭部见一类圆形软组织密度影。病理示瘤组织主要由上皮及间叶成分构成,局部可见腺管状结构,部分管腔内可见黏液,呈现多形性腺瘤表现,免疫组化提示CK、S-100、SMA等呈阳性。术后随访12个月,复查鼻内镜示左侧鼻中隔粘膜生长良好,呈术后改变,未见复发。结论鼻中隔多性腺瘤是少见病例,其诊断主要依靠术后病理及免疫组化检测。目前首选治疗方式仍然是外科手术将其切除。
Objective To investigate the clinical features,diagnosis,treatment and prognosis of pleomorphic adenoma of nasal septum so as to improve the awareness and treatment level of doctors for avoidance of misdiagnose and missed diagnosis.Methods Clinical date of one patient suffering from pleomorphic adenoma of nasal septum were analyzed retrospectively with literature review.Results Computer tomographic(CT)scans reveled an oval mass in forepart of the left nasal septum.The microscopic finding showed numbers of myoepithelial cells and duct-like structures consisting of loose myxoid stroma.This lesion had histological characteristics of a pleomorphic adenoma,and this was confirmed by immunohistochemical expression of cytokeratin,S-100 protein and SMA.The patient post-operative course was uneventful,and he is currently free from the disease 6 months after surgery.Conclusion Pleomorphic adenoma of nasal septum is very rare and diagnosis mainly depends on pathology and immunohistochemistry.Radical resection is the preferred treatment method.
