摘要
目的探讨中耳胆脂瘤多层螺旋CT及MPR重建图像的表现及其其临床价值。方法收集我院经手术后病理证实的中耳胆脂瘤患者36例的临床及影像学资料,术前患者均接受多层螺旋CT检查及MPR重建。结果36例胆脂瘤型中耳炎患者中,24例盾板变钝,18例上鼓室外侧壁破;5例听骨链缺失;2例可见锤骨移位,4例砧骨缺失,砧骨长脚缺失15例,7例锤骨长脚见部分缺失;18例鼓窦入口处明显扩大;边缘清楚的扩大骨腔隙11例;23例中耳及其周围骨结构破坏。CT表现:表现上鼓室盾板变钝或严重破坏,上鼓室扩大,听小骨消失,锤骨周围有软组织充填的密度影,骨质破坏,蒲氏间隙增宽,窦腔破坏区周边硬化。结论多层螺旋CT检查及MPR重建技术能准确显示中耳胆脂瘤的细微影像特征,对中耳胆脂瘤的诊断和鉴别诊断及手术具有十分重要的指导意义。
Objective To study the performance of multi-layer helical CT and MPR reconstruction images of ear biliary lipid tumors and to explore its clinical value.Methods Collected from January 2016 to October 2019,36 patients with midear biliary lipid tumor were confirmed after surgery,including 14 cases of female,22 cases of men,10 cases in the right ear,16 cases in the left ear,age 12-70 years old,2 to 36 years of disease.The CT performance was analyzed in combination with the surgical control.Results In 36 patients with biliary lipid tumor type otitis,24 cases of shield bluntness,18 cases of the combined outside sidewall of the drum,5 cases of total loss of hearing bone chain,2 cases of hammer bone shift,4 cases of bone bone completely missing,15 cases missing in the long foot of the bone,7 cases missing from the long foot of the hammer bone,18 cases of drum sinus intensus,11 cases of enlarged bone gap on the edge of the clear bone gap;There were 23 cases of damage to the middle ear and surrounding bone structure.CT performance:performance on the drum room shield plate blunt or serious damage,the upper drum room expansion,listen to the small bone disappear,the hammer bone around the soft tissue filled density shadow,bone damage,gap widening,sinus cavity damage area surrounding hardening.Conclusions Multi-layer helical CT examination and MPR reconstruction technology can accurately display the subtle image characteristics of mid-ear biliary lipid tumor,which is of great significance for the diagnosis and differential diagnosis and operation of mid-ear biliary lipid tumor.
