摘要
目的:探讨椎体多发转移瘤放疗中单射野中心技术应用的可行性。方法:选取10例于某院接受椎体多发转移瘤调强放疗患者,基于医科达Precise医用直线加速器与CMS XiO 4.80计划系统,以4000 cGy/20f处方剂量分别对同一患者设计2种放疗计划:plan-1采用多射野中心,plan-2采用单射野中心。利用SPSS 18.0统计学软件,比较2种计划剂量学参数及治疗参数的差异。结果:2种计划均满足靶区剂量覆盖要求,靶区D2%、Dmean、D98%、V100%、适形度指数(conformity index,CI)及均匀性指数(homogeneity index,HI)均无统计学差异(P>0.05)。2种计划双侧肺V20、脊髓Dmax、心脏Dmean及双侧肾脏V10亦无统计学差异(P>0.05),plan-2的双侧肺V5、Dmean及双侧肾脏Dmean分别比plan-1下降41.8%、18.7%和32.8%,差异具有统计学意义(P<0.05)。2种计划机器跳数、子野个数及出束时间均无统计学差异(P>0.05),plan-2的摆位时间比plan-1缩短53.1%,差异具有统计学意义(P<0.05)。结论:单射野中心技术在保证靶区剂量覆盖的条件下,有效降低了椎体多发转移瘤放疗中双侧肺V5、Dmean及双侧肾脏Dmean,并且大幅缩短了摆位时间,建议在临床应用中运用该技术。
Objective To explore the feasibility of single-isocenter technology in radiotherapy for multiple vertebral metastatic tumor.Methods Ten patients with multiple vertebral metastatic tumor receiving IMRT were enrolled in the study.Two plans with the same prescription dose of 4000 cGy/20f were designed based on Elekta Precise linear accelerator and CMS XiO 4.80 treatment planning system.Multiple-isocenter technology was applied in plan-1,and single-isocenter technology was applied in plan-2.The dosimetric parameters and treatment parameters were compared between plan-1 and plan-2 based on SPSS 18.0.Results Both the plans met target dose requirement.The D2%,Dmean,D98%,V100%,CI and HI of PTV showed no significant differences(P>0.05).The V20 of lungs,Dmax of spinal cord,Dmean of heart and V10 of kidneys showed no significant differences either(P>0.05).The V5,Dmean of lungs and Dmean of kidneys of plan-2 dropped by 41.8%,18.7%and 32.8%than plan-1(P<0.05).The monitor unit,number of segments and irradiation time showed no significant differences(P>0.05),while setup time for the patients'position of plan-2 decreased by 53.1%than plan-1(P<0.05).Conclusion The single-isocenter technology can effectively reduce V5,Dmean of lungs and Dmean of kidneys without affecting the target dose coverage,and shortens the setup time for the patients'position sharply,thus is recommended for clinical application.
