摘要
目的研究泌尿系结石并发脓毒血症的相关危险因素及措施。方法方便选取该院泌尿系结石并发脓毒血症患者60例(2012年2月—2019年2月)作为观察组,另选同期泌尿系结石未并发脓毒血症患者72例作为对照组。收集两组年龄、性别及临床特征等资料,分析泌尿系结石并发脓毒血症相关危险因素。结果合并糖尿病、术前尿培养阳性、结石直径、性别、手术时间、既往感染史是泌尿系结石并发脓毒血症的影响因素(P<0.05);Logistic多因素回归分析可知,合并糖尿病(OR=3.436,95%置信区间1.226~4.408)、术前尿培养阳性(OR=8.810,95%置信区间1.774~10.029)、手术时间>60 min(OR=2.793,95%置信区间1.506~5.724)、女性(OR=1.758,95%置信区间1.157~3.902)是泌尿系结石并发脓毒血症的独立危险因素(P<0.05)。结论合并糖尿病、术前尿培养阳性、手术时间>60 min、女性是泌尿系结石并发脓毒血症的独立危险因素,临床应根据上述因素采取个体化、针对性防治措施,以避免或减少脓毒血症发生。
Objective To study the related risk factors and measures of urinary calculi complicated with sepsis.Methods Sixty patients with convenient urinary calculi complicated with sepsis(February 2012 to February 2019)were enrolled in the study group.72 patients with urinary calculi without sepsis were selected as the control group.Data such as age,gender and clinical characteristics were collected to analyze the risk factors associated with urinary calculi complicated with sepsis.Results Diabetes mellitus,preoperative urine culture positive,stone diameter,gender,operation time,and previous infection history were the influencing factors of urinary calculi complicated with sepsis(P<0.05).Logistic multivariate regression analysis showed that diabetes was associated with diabetes(OR=3.436,95% confidence interval 1.226~4.408),preoperative urine culture positive(OR=8.810,95% confidence interval 1.774~10.029),operation time>60 min(OR=2.793,95% confidence interval 1.506~5.724),female(OR=1.758,95%confidence interval 1.157 to 3.902)was an independent risk factor for urinary calculi complicated with sepsis(P<0.05).Conclusion Complications of diabetes,positive preoperative urine culture,>60 min of operation time,and females are independent risk factors for urinary tract calculi complicated with sepsis.According to the above factors,individualized and targeted prevention and treatment measures should be taken clinically to avoid or reduce the occurrence of sepsis.
