摘要
目的 探讨螺旋CT联合红外光谱自动分析系统在肾结石成分分析中的诊断价值及临床意义。方法 回顾性分析2012年6月-2018年10月本院收治的472例肾结石患者的临床资料,其中男271例,女201例。术前患者均行CT检查,术后采用红外光谱自动分析系统进行结石成分分析,回顾性测定各类型结石CT值,比较各类型结石CT值之间差异。结果 红外光谱自动分析结果:单纯性肾结石165例(35.0%),以草酸钙结石(77.0%)为主;混合性肾结石307例(65.0%),以一水草酸钙+二水草酸钙(30.3%)为主。一水草酸钙结石CT值(1272±151) HU、感染性结石CT值(785±128) HU、无水尿酸CT值(593±109) HU,单纯性结石CT值之间比较差异均有统计学意义(P<0.05);混合性结石CT值均介于相应纯结石CT值之间。结论 术前通过螺旋CT值测定可初步判断结石成分,术后红外光谱自动分析系统可定性分析结石成分;螺旋CT联合结石红外光谱自动分析系统对肾结石诊断、治疗及预防具有一定的临床实用价值。
Objective To explore the diagnostic value and clinical significance of spiral CT combined with infrared spectrum automatic analysis system in component analysis of renal calculi.Methods The clinical data of 472 patients with renal calculi admitted to our hospital from June 2012 to October 2018 were retrospectively analyzed.There were 271 males and 201 females.Before operation,all patients underwent CT examination.After the operation,the infrared spectrum automatic analysis system was used to conduct the component analysis of renal calculi.CT values of all kinds of types of calculi were determined retrospectively.The differences between CT values of all kinds of types of calculi were compared.Results Infrared spectrum automatic analysis results:There were 165 cases(35.0%)of simple renal calculi.They were mainly calcium oxalate calculi(77.0%).There were 307 cases(65.0%)of mixed renal calculi.They were mainly calcium oxalate monohydrate plus calcium oxalate dihydrate(30.3%).CT value of calcium oxalate monohydrate calculi was(1272±151)HU,CT value of infectious calculi was(785±128)HU and it was(593±109)HU for anhydrous uric acid.The difference between CT values of simple calculus was statistically significant(P<0.05).CT values of mixed calculi were all between the corresponding CT values of pure calculi.Conclusions Before operation,the calculi composition could be preliminarily judged by spiral CT value measurement.The post-operative infrared spectrum automatic analysis system could qualitatively analyze calculi components.Spiral CT combined with calculi infrared spectrum automatic analysis system has certain clinical practical value for diagnosis,treatment and prevention of kidney calculi.
