摘要
目的探讨血管粘附分子-1(VCAM-1)和粘附分子P-选择素在脑膜瘤中表达及临床意义。方法随机选取脑膜瘤患者78例,作为脑膜瘤组,另随机选取同期健康体检人员78例作为对照组,应用免疫组织化学及PCR对两组人员的VCAM-1、P-选择素表达水平进行检测,然后统计分析两组人员的VCAM-1、P-选择素表达水平、阳性情况,并分析脑膜瘤组患者的VCAM-1、P-选择素表达与各临床参数的相关性。结果脑膜瘤组患者的VCAM-1、P-选择素表达水平均显著高于对照组(P<0.05),阳性率[43.6%(34/78)、53.8%(42/78)]均显著高于对照组(0、0)(P<0.05)。有硬脑膜破坏、硬膜尾征、侵袭脑组织/软脑膜/蛛网膜患者的VCAM-1、P-选择素阳性表达率显著高于无硬脑膜破坏、硬膜尾征、侵袭脑组织/软脑膜/蛛网膜患者(P<0.05),但不同性别、年龄、临床分级、颅骨破坏、颅骨增生患者的VCAM-1、P-选择素阳性表达率之间的差异均不显著(P>0.05)。结论VCAM-1和粘附分子P-选择素在脑膜瘤中表达水平、阳性率均提升,在脑膜瘤侵袭性的判定中可以作为参考指标。
Objective To investigate the expression and clinical significance of vascular adhesion molecule-1(VCAM-1)and P-selectin in meningiomas.Methods 78 patients with meningioma were randomly selected as the meningioma group,and 78 healthy persons in our hospital during the same period were randomly selected as the control group.The expression levels of VCAM-1 and P-selectin in the 2 groups were detected by immunohistochemistry and PCR,and then the expression levels of VCAM-1 and P-selectin in the 2 groups were statistically analyzed.The expression of VCAM-1 and P-selectin in patients with meningioma and the correlation between the expression of VCAM-1 and P-selectin and clinical parameters were analyzed.ResultsThe expression levels of VCAM-1 and P-selectin in meningioma group were significantly higher than those of the control group(P<0.05).The positive rates of VCAM-1 and P-selectin in meningioma group were 43.6%(34/78)and 53.8%(42/78)respectively,which were significantly higher than those of the control group(P<0.05).The positive expression rates of VCAM-1 and P-selectin in patients with dural destruction,dural tail sign,invasive brain tissue/pia mater/arachnoid membrane were significantly higher than those in patients without dural destruction,dural tail sign,invasive brain tissue/pia mater/arachnoid membrane(P<0.05),However,there were no significant differences in the positive expression rates of VCAM-1 and P-selectin between different sex,age,clinical grade,skull destruction and skull hyperplasia(P>0.05).Conclusion The expression level and positive rate of VCAM-1 and P-selectin in meningiomas are increased,which can be used as a reference index in judging the invasiveness of meningiomas.
