摘要
目的探讨分析中药配合具有中医特色的综合护理对慢性乙肝肝硬化气滞血瘀型患者治疗效果及生活质量的影响。方法抽取2016年6月至2019年6月无锡市第五人民医院收治的慢性乙肝肝硬化气滞血瘀型患者130例,按随机数字表分为对照组和研究组,对照组患者住院期间给予常规治疗与护理,研究组患者在此基础上给予中药调理及具有中医特色的综合护理方案,连续干预4周。比较两组患者治疗护理前后肝功能指标变化和临床症状;通过生存质量测定量表简表(WHOQOL-BREF)评估两组患者生存质量;比较两组患者对护理服务满意情况。结果干预4周后,研究组患者中医证候积分和谷草转氨酶、谷丙转氨酶以及总胆红素水平均明显低于对照组(均P<0.05);WHOQOL-BREF表各维度评分、护理满意度(98.46%vs 84.62%)、整体治疗效果均显著优于对照组(均P<0.05)。结论对慢性乙肝肝硬化气滞血瘀型患者应用中药调理及具有中医特色综合护理方案进行治疗,可以有效促进患者临床症状消退、肝功能改善,提高患者生活质量,同时整体提升临床护理质量,护理效果显著。
Objective To investigate the effect of traditional Chinese medicine combined with comprehensive nursing with the characteristics of Chinese medicine on the curative effect and life quality of patients with chronic HBV cirrhosis,qi stagnation and blood stasis.Methods A total of 130 patients with chronic HBV cirrhosis,liver stagnation and spleen deficiency type were selected from January 2017 to June 2018 in the Fifth People s Hospital of Wuxi City.The patients were divided into control group and study group by the method of random number table.Patients in the control group were given conventional treatment and nursing care during hospitalization.Based on the interventions in control group,the patients in study group were treated with Chinese medicine and comprehensive nursing plan with Chinese medicine characteristics for 4 weeks.The changes of liver function indexes and clinical symptoms before and after nursing were compared between the two groups of patients as well as the quality of life assessed by the WHOQOL-BREF.The satisfaction rates of the two groups of patients on nursing services were compared as well.Results After 4 weeks of intervention,the Chinese medicine syndrome scores,the levels of aspartate aminotransferase,alanine aminotransferase and total bilirubin in the study group were significantly lower than those of the control group(all P<0.05).The scores of WHOQOL-BREF in all dimensions and nursing satisfaction rate of the patients in study group were significantly higher than those of the control group(all P<0.05).Conclusions The treatment of patients with chronic h-epatitis B cirrhosis,qi stagnation and blood stasis with traditional Chinese medicine treatment and comprehensive nursing program with Chinese medicine characteristics can effectively relief the disappearance of clinical symptoms and improve liver function and life quality of patients.
