摘要
目的观察扶正解毒方联合针刺治疗结直肠癌术后化疗患者癌因性疲乏疗效及对外周血T细胞亚群、血清免疫球蛋白水平的影响。方法将100例结直肠癌术后化疗患者随机分为2组,其中对照组50例给予最佳支持治疗,观察组50例则在此基础上加用扶正解毒方合针刺治疗,观察2组治疗前后Piper疲乏自评量表、生活质量核心问卷(QLQ-C30)、气虚血瘀症候评分变化情况,检测2组外周血T细胞亚群及血清免疫球蛋白IgA、IgG、IgM水平。结果2组治疗后Piper疲乏自评量表各项评分、QLQ-C30中疲乏评分、气虚血瘀症候评分均明显降低(P均<0.05),QLQ-C30中其余各项评分均明显升高(P均<0.05),且观察组各评分改善情况均明显优于对照组(P均<0.05);治疗后观察组T细胞亚群中CD8+水平显著低于治疗前及对照组(P均<0.05),CD3+、CD4+、NK及免疫球蛋白水平均显著高于治疗前及对照组(P均<0.05),对照组治疗后各免疫指标未见明显变化(P均>0.05)。结论扶正解毒方联合针刺用于结直肠癌术后化疗患者可有效缓解癌因性疲乏,减轻相关中医证候,改善生存质量,并有助于调节T淋巴细胞亚群和免疫球蛋白水平。
Objective It is to observe the effect of Fuzheng Jiedu decoction combined with acupuncture on cancer-related fatigue in patients after postoperative chemotherapy for colorectal cancer and its effect on the levels of peripheral blood T cell subsets and serum immunoglobulins.Methods 100 patients with colorectal cancer treated with postoperative chemotherapy in Dongguan Hospital of Traditional Chinese Medicine were randomly divided into 2 groups,in which 50 patients in the control group were given the best supportive treatment,and 50 patients in the observation group were treated with Fuzheng Jiedu decoction and acupuncture based on the control group.The changes of scores of Piper fatigue self-assessment scale,core quality of life questionnaire(QLQ-C30),and syndrome of Qi deficiency and blood stasis were observed,the levels of peripheral blood T cell subsets and serum immunoglobulin IgA,IgG,IgM were measured before and after treatment in the two groups.Results The scores of Piper fatigue self-assessment scale,fatigue score in QLQ-C30,and syndrome of Qi deficiency and blood stasis were significantly reduced(P<0.05),and the other scores in QLQ-C30 were significantly increased after treatment in the two groups(P<0.05),and the improvement in the observation group was significantly better than that in the control group(P<0.05);the levels of CD8+in the T cell subsets of the observation group were significantly lower than those before treatment and the control group(P<0.05),and CD3+,CD4+,NK and immunoglobulin were significantly higher than before treatment and in the control group(P<0.05).Conclusion Fuzheng Jiedu decoction combined with acupuncture can effectively relieve cancer-related fatigue,relieve TCM syndromes,improve quality of life,and help to regulate T lymphocyte subsets and immunoglobulin levels in patients with colorectal cancer after postoperative chemotherapy.
