摘要
目的研究电刺激治疗急性中风冈上肌对上肢功能恢复的疗效。方法选取2016年1月至2018年4月本院收治的120例急性脑卒中后有上肢功能障碍的患者,随机分为对照组和实验组,各60例。主要观察时间点为发病后7 d,实验组给予低频脉冲电刺激治疗冈上肌,同时给予Bobath治疗技术、Rood治疗技术、强制性使用运动治疗技术、运动再学习技术、以功能为导向的运动治疗技术、功能性电刺激治疗技术以及针灸治疗;对照组给予Bobath治疗技术、Rood治疗技术、强制性使用运动治疗技术、运动再学习技术、以功能为导向的运动治疗技术、功能性电刺激治疗技术以及针灸治疗,治疗疗程为20 d,每天2次,每次30 min,通过神经电生理检查神经传导速度、外展肌力、上肢功能、手功能。结果治疗20 d后,两组治疗前后FMA-UE、RAHFT、ADL、FIM评分及肩关节表面肌电检测(积分肌电值iEMG)结果及组间比较差异均具有统计学意义(P<0.05)。结论电刺激治疗急性中风冈上肌能促进上肢功能的恢复,改善手功能,提高其生活质量。
Objective To study the effect of electrical stimulation on upper limb function recovery in acute apoplexy patients.Methods 120 patients with upper limb dysfunction after acute stroke from January 2016 to April 2018 were randomly divided into control group and experimental group,with 60 cases in each group.The main observation time was 7 days after onset,the experimental group was treated with low frequency pulsed electrical stimulation for supraspinatus muscle,at the same time,Bobath therapy,Rood therapy,compulsory exercise therapy,motor relearning,function-oriented exercise therapy,functional electrical stimulation therapy and acupuncture treatment were given.The course of treatment was 20 days,twice a day,30 minutes each time,The evaluation indexes were nerve conduction velocity,abductor muscle strength,upper limb function,hand function by electrophysiological examination.Results After 20 days of treatment,the results of fma-ue,RAHFT,ADL,FIM score,shoulder surface emg(integral emg value)and the comparison between the two groups before and after treatment were statistically significant(P<0.05).Conclusion Electric stimulation can promote the recovery of upper limb function,improve hand function and improve the quality of life in the treatment of acute apoplexy.
