摘要
目的观察分析温阳健脾汤辅助PPH术治疗直肠黏膜内脱垂的临床效果。方法选取2016年6月至2018年6月本院手术治疗的直肠黏膜内脱垂患者60例,根据治疗先后均分为两组,各30例。两组患者均行PPH术治疗,对照组予术后常规治疗,观察组在此基础上配合温阳健脾汤辅助治疗。比较分析两组患者术后恢复情况、症状评分及治疗总有效率,均随访6个月记录复发情况和脾肾阳虚证候评分。结果观察组治疗总有效率93.33%(28/30)与对照组90.00%(27/30)比较,差异无统计学意义(P>0.05);观察组术后第5天肛门坠胀、排尿困难、大便出血、VAS评分分别为(29.25±6.54)分、(28.36±6.11)分、(27.08±5.45)分、(4.05±0.98)分,均低于对照组,差异均有统计学意义(P<0.05);治疗前,两组患者中医证候评分比较,差异无统计学意义(P>0.05);治疗后,观察组中医证候评分为(7.34±3.21)分,低于对照组,差异有统计学意义(P<0.05);随访6个月,观察组肛门坠胀、失禁、狭窄等发生率、复发率分别为3.33%、0.00%,均低于对照组20.00%、13.33%,差异均有统计学意义(均P<0.05)。结论直肠黏膜内脱垂患者行PPH术后应用温阳健脾汤辅助治疗可有效改善临床症状,降低并发症发生率和复发率,对术后康复有积极促进作用。
Objective To observe the clinical effect of Wenyang Jianpi Decoction and PPH in the treatment of rectal mucosal prolapse.Methods A total of 60 patients with rectal mucosal prolapse who underwent surgery from June,2016 to June,2018 in our hospital were enrolled.According to the treatment order,they were divided into a control group and an observation group,with 30 cases in each group.Both groups were treated with PPH.After the surgery,the control group received routine treatment;in addition,the observation group was treated with Wenyang Jianpi Decoction.The postoperative recovery,symptom scores,and total effective rate were compared between the two groups.The recurrence and spleen-kidney yang deficiency syndrome scores were recorded during 6 months follow-up.Results The total effective rate was 93.33%(28/30)in the observation group,and was 90.00%(27/30)in the control group,with no statistical difference(P>0.05).the scores of anus bulge,dysuria,stool bleeding,and VAS were(29.25±6.54),(28.36±6.11),(27.08±5.45),and(4.05±0.98)in the observation group,which were lower than those in the control group(P<0.05).Before the treatment,there was no statistical difference in the score of Chinese medicine symptoms between the two groups(P>0.05).After the treatment,the score of Chinese medicine symptoms was(7.34±3.21)in the observation group,which was lower than that in the control group(P<0.05).The incidence of anal bulge,incontinence,and stenosis as well as the recurrence rate were significantly lower in the observation group than in the control group during the follow-up period(3.33%vs.20.00%and 0.00%vs.13.33%;both P<0.05).Conclusion Wenyang Jianpi Decoction and PPH for patients with rectal mucosal prolapse can effectively improve their clinical symptoms and postoperative rehabilitation,and reduce the incidence of complications and recurrence rate.
