摘要
目的探讨乙型肝炎病毒脱氧核糖核酸(HBV-DNA)水平与乙型肝炎免疫标志物五项指标之间的关系,探讨乙肝五项联合HBV-DNA检测在乙型肝炎病毒(HBV)感染诊断中的应用价值。方法分析该院感染科门诊和住院部2017年1月至2017年12月收治的529例乙型肝炎患者,对所有患者进行乙肝五项检查与HBV-DNA定量。结果529例乙型肝炎患者血清乙肝五项指标检查结果存在不同指标阳性,主要以HBsAg(+)、HBeAg(+)、HBcAb(+)和HBsAg(+)、HBeAg(-)、HBcAb(+)这两种模式为主,前者HBV-DNA阳性率高于后者,差异有统计学意义(P<0.05);HBeAg与HBV-DNA之间的关联性明显(P<0.01),HBeAg(+)患者的HBV-DNA阳性率明显高于HBeAg(-)患者(P<0.05)。不同的HBV-DNA含量患者的HBeAg阳性率也不相同,当病毒含量达到一定含量(>10^6)时,病毒含量越高,HBeAg的阳性率越高(P<0.05)。结论HBV-DNA检查结果可用于反映病毒是否出现复制情况,采用乙肝五项与HBV-DNA联合检测能准确对乙型肝炎进行诊断,对乙型肝炎患者后续治疗提供了科学、有利的依据,值得临床推广应用。
【Objective】To investigate the relationship between HBV-DNA level and five indexes of hepatitis B immune markers,and to explore the value of hepatitis B combined with HBV-DNA in the diagnosis of hepatitis B virus(HBV)infection.【Methods】A total of 529 cases of hepatitis B admitted from January 2017 to December 2017 in the outpatient department and inpatient department of the department of infectious diseases in our hospital were analyzed,and five HBV examinations and HBVDNA quantitative tests were performed on all patients.【Results】There were positive results of five indexes of hepatitis B in 529 cases,mainly based on HBsAg(+),HBeAg(+),HBcAb(+)and HBsAg(+),HBeAg(-),HBcAb(+);HBV-DNA positive rate of the former was higher than the latter,with significant differences(P<0.05).The correlation between HBeAg and HBV-DNA was obvious(P<0.01);the HBV-DNA positive rate in HBe Ag(+)patients was significantly higher than that in HBe Ag(-)patients(P<0.05).The positive rate of HBeAg in patients with different HBV-DNA levels was also different;when the virus content reaches a certain level(>10^6),the higher the virus content,the higher the positive rate of HBeAg(P<0.05).【Conclusion】The HBV-DNA test results can be used to reflect whether the virus has been replicated.The combined detection of hepatitis B and HBV-DNA can accurately diagnose hepatitis B,and provide a scientific and favorable basis for subsequent treatment of hepatitis B patients.It is worth clinical promotion.
