摘要
目的·探讨血清25-羟基维生素D[25-hydroxyvitamin D,25(OH)D]水平与脑白质疏松症(leukoaraiosis,LA)之间的相关性。方法·选取2017年10月-2018年10月在无锡市第五人民医院神经内科就诊的LA患者170例,收集患者一般临床资料并测定血清25(OH)D水平。行头颅磁共振成像评估LA的严重程度。根据Fazekas分级将LA患者分为4组,比较不同组间25(OH)D水平的差异。应用相关性分析及多因素Logistic回归分析研究血清25(OH)D水平与LA严重程度的关系。结果·随着LA严重程度的增加,25(OH)D水平呈下降趋势,差异具有统计学意义(P=0.000)。相关性分析结果显示,25(OH)D与LA严重程度呈负相关(r=-0.534,P=0.000)。多因素Logistic回归分析结果显示,LA病变的危险因素为年龄增加(OR=1.108,95%CI 1.054~1.165,P=0.000)、高血压(OR=2.894,95%CI 1.060~7.904,P=0.038)和25(OH)D水平下降(OR=0.949,95%CI 0.925~0.974,P=0.000)。结论·血清25(OH)D水平与LA的严重程度具有相关性,是LA的危险因素。
Objective·To investigate the correlation between serum 25-hydroxyvitamin D[25(OH)D]level and leukoaraiosis(LA).Methods·A total of 170 patients with LA who underwent treatment in the Department of Neurology in Wuxi Fifth People’s Hospital from Oct.2017 to Oct.2018 were enrolled.Their general clinical data were collected and the serum 25(OH)D levels were measured.Head cerebral magnetic resonance imaging was performed to assess the severity of LA.The patients were divided into four groups according to Fazekas scale and the serum 25(OH)D levels of patients in different groups were compared.Correlation analysis and multivariate Logistic regression analysis were used to study the relationship between serum 25(OH)D level and the severity of LA.Results·With the increase of the severity of LA,the serum 25(OH)D levels showed a downward trend,and the difference was statistically significant(P=0.000).Correlation analysis showed that serum 25(OH)D levels were negatively correlated with the severity of LA(r=-0.534,P=0.000).Multivariate Logistic regression analysis showed that the risk factors for LA were increased age(OR=1.108,95%CI 1.054–1.165,P=0.000),hypertension(OR=2.894,95%CI 1.060–7.904,P=0.038)and decreased 25(OH)D level(OR=0.949,95%CI 0.925–0.974,P=0.000).Conclusion·Serum 25(OH)D level is associated with the severity of LA,and is a risk factor for LA.
