摘要
目的探讨HEART评分及血浆D-二聚体水平对急性冠脉综合征心血管不良事件的风险预测价值.方法抽取山西大同大学附属医院2017年1月至2019年1月收治的100例急性冠脉综合征患者纳入研究.将患者按照HEART评分进行评价分层,随访患者在发病30 d内的心血管不良事件发生情况,依据其工作特性曲线分析患者血浆D-二聚体水平与HEART评分的预测效能,并比较HEART评分对急性冠脉综合征患者发病30 d内发生心血管不良事件的预测效能.结果急性冠脉综合征患者发病30 d内发生心血管不良事件21例,占比为21.00%(21/100).联合血浆D-二聚体水平后的HEART评分相对与单一HEART评分法对预测急性胸痛患者心血管事件的灵敏度方面有显著优势,且其假阴性率低于单一HEART评分,其准确率也高于单一HEART预测方法,差异有统计学意义(P<0.05).结论在急性冠脉综合征患者的心血管不良事件预测中,采用HEART评分联合血浆D-二聚体可在一定程度上提高其对风险预测的价值.
Objective To investigate the predictive value of HEART score combined with D-dimer in the treatment of acute coronary syndromes for cardiovascular adverse events.Methods A total of 100 patients with acute coronary syndrome admitted to the Affiliated Hospital of Shanxi Datong University from January 2017 to January 2019 were included in the study.Patients were stratified according to their HEART scores.The patients were followed up for cardiovascular adverse events within 30 days of onset.The predictive efficacy of HEART scores and the D-dimer level were analyzed according to receiver operating characteristic curves.Predictive efficacy of HEART score combined with D-dimer level was compared with single HEART score for cardiovascular adverse events within 30 days.Results There were 21 cases of cardiovascular adverse events within 30 days of acute coronary syndrome onset,accounting for 21.00%.DL-dimer combined with HEART score after the polymer level was significantly superior to the single HEART score in predicting the cardiovascular events in patients with acute chest pain,with greater sensitivity,and its false negative rate is significantly lower than the single HEART score,and its accuracy was higher than that of the single HEART score.Conclusions The HEART score combined with D-dimer in the prediction of cardiovascular adverse events in patients with acute coronary syndrome can improve its risk prediction value to some extent.
