摘要
目的:观察五子衍宗丸联合十一酸睾酮胶丸治疗男性2型糖尿病合并迟发性性腺功能减退症的临床疗效。方法:将86例患者随机分为2组,每组42例,2组维持原有控制血糖、血压、调整血脂等基础治疗,对照组服用十一酸睾酮胶丸,80 mg/次,2次/d,2周之后改为40 mg/次,2次/d。观察组在此基础上加用五子衍宗丸,6 g/次,2次/d。2组均连续治疗6个月。观察2组治疗前后一般项目、糖脂代谢治疗、性激素水平、中医症状积分以及安全性指标。结果:对照组治疗后空腹血糖(FBG)、餐后2小时血糖(2h PBG)、糖化血化蛋白(Hb A1C)、总胆固醇(TC)、低密度脂蛋白(LDL-C)、胰岛素抵抗指数(HOMA-IR)较治疗前显著改善,差异有统计学意义(P<0.05);观察组体质指数、腰围、FBG、2h PBG、Hb A1C、HOMA-IR、TC、LDL-C、高密度脂蛋白(HDL-C)、甘油三酯(TG)均较治疗前及对照组明显改善,差异有统计学意义(P<0.05)。治疗前2组总睾酮(TT)、性激素结合球蛋白(SHBG)、黄体生成素(LH)、卵泡刺激素(FSH)比较,差异无统计学意义(P>0.05);治疗后2组TT水平较治疗前均有显著提高,差异有统计学意义(P<0.05),组间比较观察组改善更明显,差异有统计学意义(P<0.05);治疗前后2组SHBG、LH、FSH变化不明显,差异无统计学意义(P>0.05)。中医症状积分治疗前后2组组内比较,差异有统计学意义(P<0.05);治疗后组间比较差异也有统计学意义(P<0.05)。2组治疗后无不良反应发生,肝、肾功能、血常规未见异常。结论:五子衍宗丸联合十一酸睾酮胶丸治疗男性2型糖尿病合并迟发性性腺功能减退症疗效显著。
Objective:To observe clinical effects of Wuzi Yanzong pills and testosterone undercanoate soft capsules in the treatment for male patients suffering type two diabetes mellitus(T2 DM)complicated by late-onset hypognonadism(LOH).Methods:All 86 patients were randomized into the control group and the observation group,42 cases each group,both groups mantained the basic therapy of controlling blood glucose,blood pressure,adjusting blood lipid and others,and the control group took testosterone undercanoate soft capsules,80 mg each time,twice each day,reducing to 40 mg each time in two weeks,twice each day.The observation group took Wuzi Yanzong pills based on testosterone undercanoate soft capsules,6 g each time,twice each day.Both groups were treated for six months consecutively.To observe the general items,glycolipid metabolism treatment,sex hormone level,TCM symptom scores and safety indexes before and after treating in both groups.Results:FBG,2 h PBG,Hb A1 c,TC,LDL-C and HOMA-IR of the control group after treating improved compared with before treating,and the difference had statistical meaning(P<0.05);constitutional index,waistline,FBG,2 h PBG,Hb A1 c,HOMA-IR,TC,LDL,HDL-C and TG of the observation group improved compared with before treating and the control group notably,and the difference could be found(P<0.05).The difference had no statistical meaning in TT,SHBG,LH and FSH between both groups before treating(P>0.05);after treating,TT levels raised compared with before treating notably in both groups,and the difference showed statistical meaning(P<0.05),the improvements of the observation group were more notable,and the difference showed statistical meaning(P<0.05).the changes of SHBG,LH and FSH in both groups were insignficant before and after treating,and the difference had no statistical meaning(P>0.05).The difference had statistical meaning in TCM symptoms scores before and after treating within two groups(P<0.05);the difference showed statistical meaning between both groups after treating(P<0.05).No adverse
