摘要
目的分析PFNA-Ⅱ治疗老年不稳定型股骨粗隆间骨折的临床疗效,为临床选择最佳诊疗方式提供依据。方法选取我院2016年3月-2018年3月收治的不稳定型股骨粗隆间骨折老年患者84例为研究对象,随机分为治疗组和对照组各42例,对照组患者采用股骨近端解剖锁定钢板治疗,治疗组患者采用PFNA-Ⅱ治疗,比较两组患者手术效果。结果治疗组患者手术时间、切口长度、术中出血量、术后引流量、下地负重时间均明显低于对照组,差异显著(P <0. 05);术后1、3及6个月,治疗组髋关节功能评分明显高于对照组,差异显著(P <0. 05);术后12个月两组髋关节功能评分比较差异无统计学意义(P>0. 05);治疗组术后并发症发生率为4. 76%,明显低于对照组21. 43%,两组比较差异显著(P <0. 05)。结论采用PFNA-Ⅱ治疗老年不稳定型股骨粗隆间骨折疗效确切,具有手术时间短、出血少、创伤小、术后短期恢复快的优点,且可有效降低术后并发症,值得临床推广应用。
Objective To analyze the clinical efficacy of PFNA-Ⅱ in the treatment of unstable intertrochanteric fractures in the elderly and provide a basis for clinical selection of the best diagnosis and treatment. Methods 84 elderly patients with unstable intertrochanteric fractures from March 2016 to March 2018 enrolled in our hospital were randomly divided into a treatment group and a control group with 42 in each group. The patients in the control group were treated with anatomical locking plate while those in the treatment group were treated with PFNA-Ⅱ,and the surgical Results of the two groups were compared. Results The operation time,incision length,intraoperative blood loss,postoperative drainage volume and weight loss time of the treatment group were significantly lower than those of the control group,and the difference was significant( P < 0. 05). After 1,3 and 6 months,the hip function score of the treatment group was significantly higher than that of the control group( P < 0. 05). There was no significant difference in hip function score between the two groups 12 months after operation( P > 0. 05). The postoperative complication rate of the treatment group stood at 4. 76%,significantly lower than that of the control group,which stood at 21. 43%,and the difference was significant( P < 0. 05). Conclusion PFNA-Ⅱ is effective in the treatment of unstable intertrochanteric fractures in the elderly. It has the advantages of short operation time,less bleeding,less trauma,and short-term recovery after surgery. It can effectively reduce postoperative complications and is thus worthy of clinical application.
