声触诊组织定量分析评估孕中晚期宫颈软硬度的价值 被引量:3

摘要 目的通过测量剪切波速度(SWV)评估孕中晚期宫颈的软硬度,为预测早产、正确选择分娩方式提供客观依据。方法选取2017年10月至2018年10月在扬州大学附属医院产科就诊并最终足月生产的孕中晚期孕妇74例,孕妇年龄23~37岁,平均年龄29.27岁。依次进行灰阶超声及声触诊组织定量分析(VTQ),测量宫颈长度,记录宫颈内外口SWV,比较分析初产妇孕中晚期宫颈内外口SWV、经产妇孕中晚期宫颈内口SWV差异有无统计学意义。结果初产妇孕中期宫颈内口SWV明显高于宫颈外口[(254.4±60.6)cm/s vs(139.0±28.6)cm/s],差异有显著统计学意义(P<0.01)。初产妇孕晚期宫颈内口SWV明显高于宫颈外口[(222.2±50.4)cm/s vs(127.1±24.9)cm/s],差异有显著统计学意义(P<0.01)。经产妇顺产、剖宫产孕中期宫颈内口SWV略高于孕晚期宫颈内口[(237.5±55.5)cm/s vs(196.2±43.0)cm/s,(242.2±41.1)cm/s vs(193.3±60.2)cm/s],但差异无统计学意义(P>0.05)。初产妇孕中期及孕晚期宫颈外口均较宫颈内口SWV明显降低,差异具有显著统计学意义(P<0.01)。初产妇、经产妇孕中期及孕晚期宫颈内口SWV差异均无统计学意义(P>0.05)。孕中期及孕晚期宫颈长度均与SWV无相关性。结论VTQ所获得的SWV具有较好的可重复性,可作为评估孕中晚期宫颈软硬度的一种方法。 Objective To evaluate the softness and hardness of cervix in second and third trimesters of pregnancy by measuring the shear wave velocity(SWV),and provide objective basis for predicting premature delivery and choosing delivery mode.Methods From October 2017 to October 2018,74 second and third trimesters of pregnant were enrolled,which aged 23-37 years old with mean age of 29.27 years old.The grayscale ultrasound and virtual touch quantification(VTQ)were used to mea-sure length of cervix,and shear wave velocity(SWV)was recorded.The difference of SWV between the internal and external cervical orifice of primipara as well as internal cervical of multipara in second and third trimesters of pregnancy were compared.Results The cervix internal valve SWV in second trimesters of primipara was statistically significantly higher than that of external[(254.4±60.6)cm/s vs(139.0±28.6)cm/s;P<0.01].The cervix internal valve SWV in third trimesters of primipara was statistically significantly higher than that of external[(222.2±50.4)cm/s vs(127.1±24.9)cm/s;P<0.01].The cervical internal valve SWV in second trimesters spontaneous labor and cesarean section multipara was slightly higher than that of third trimester trimesters[(237.5±55.5)cm/s vs(196.2±43.0)cm/s,(242.2±41.1)cm/s vs(193.3±60.2)cm/s],but the difference was no statistically significant(P>0.05).The cervix external valve SWV in second and third trimesters primipara was significantly lower than that of internal,and the difference was statistically significant(P<0.01).There was no significant difference in cervix external valve SWV between second and third trimester in primipara and multipara(P>0.05).There was no correlation between cervical length and SWV in second and third trimesters of pregnancy(P>0.05).Conclusion It is demonstrated that SWV obtained by VTQ showed good repeatability,which could be used to evaluate cervical softness and hardness in second and third trimesters of pregnancy.
