摘要
目的评价针灸或针灸配合中药等方法治疗消化性溃疡(peptic ulcer,PU)的有效性、安全性。方法计算机检索2008年1月-2018年12月发表在万方数据库、中国期刊网全文数据库、重庆维普数据库、中国生物医学文献数据库、PubMed和Cochrane Library的有关针灸或其配合中药等方法治疗消化性溃疡的相关研究,对符合纳入标准的文献进行资料提取和质量评价,应用RevMan统计软件对纳入的文献数据进行Meta分析。结果①共纳入25篇文献,均为中文文献,合计2233例患者;②荟萃分析结果显示:针灸配合中药、西药相对于单纯西药疗法具有更好疗效(P<0.00001),更高的幽门螺旋杆菌清除率(P<0.00001)、更少的不良反应(P<0.00001)及更低的复发率(P<0.05),并可以提高患者生存质量。且亚组分析结果显示针刺分别配合中药、西药疗法相对于西药疗法均具有更好疗效。结论针灸配合中药、西药相对于单纯西药治疗是消化性溃疡临床治疗的更优方案,但由于此次纳入文献的质量等级不高,故需谨慎对待此结论,仍需要更多的临床大样本、多中心、高质量的随机对照试验进行进一步的研究与验证。
Objective To evaluate the efficacy and safety of acupuncture or acupuncture combined with traditional Chinese medicine in the treatment of peptic ulcer(PU). Methods Computer search for acupuncture or its combination with Chinese medicine in Wanfang database, China Journal Net full-text database, Chongqing Weipu database, China Biomedical Literature Database, PubMed and Cochrane Library from January to February 2008. Data extraction and quality evaluation of the literatures that met the inclusion criteria were performed, and meta-analysis was conducted by the RevMan statistical software. Results(1) A total of 25 articles were included, all in Chinese, with a total of 2233 patients.(2) Meta-analysis results show that acupuncture combined with traditional Chinese medicine and western medicine has better curative effect than Ps alone(P < 0.000 01), higher Helicobacter pylori(Hp) clearance rate(P <0.000 01), less Adverse reactions(P < 0.000 01), lower recurrence rate(P < 0.05), and can improve the quality of life of patients. The results of subgroup analysis showed that acupuncture combined with traditional Chinese medicine and western medicine therapy had better curative effect than western medicine therapy. Conclusion Acupuncture combined with traditional Chinese medicine and western medicine is a better treatment for peptic ulcer than pure western medicine. However, because the quality level of this paper is not high, it is necessary to treat this conclusion with caution, and still need more clinical large samples. Multi-center, high-quality randomized controlled trials for further research and validation.
