摘要
目的探究在2型糖尿病合并股骨转子间粉碎性骨折中应用带锁髓内钉与锁定钢板内固定治疗的疗效。方法选取2017年1月1日至2018年1月1日收治的股骨转子间粉碎性骨折合并2型糖尿病患者70例作为研究对象。采用随机数字表法将其随机分为A(带锁髓内钉内固定治疗)组和B(锁定钢板内固定治疗)组。观察2组患者手术指标、治疗优良率、术后1年髋关节功能、术后不同时间疼痛评分、术后1年生活质量评分和不良事件发生情况。结果A组患者手术时间、术中出血量、术后引流量、骨折愈合时间均显著小于B组,但治疗优良率显著高于B组,差异有统计学意义(P<0.05)。2组患者髋关节恢复优良率均为88.6%,差异无统计学意义(P>0.05)。2组患者术后VAS评分均显著降低,但A组优于B组,差异有统计学意义(P<0.05)。2组患者治疗后生理功能、生理职能、躯体疼痛、活力、社会功能、情感职能、精神健康以及总体健康状态评分均显著改善,但B组优于A组,差异有统计学意义(P<0.05)。2组患者术后6个月内不良事件发生情况相比,差异无统计学意义(Z=1.283,P=0.733)。结论与锁定钢板内固定相比,带锁髓内钉内固定治疗2型糖尿病合并股骨转子间粉碎性骨折患者具有,更短的手术时间和骨折愈合时间,更少的术中出血量和术后引流量以及更高的优良率,能够显著改善髋关节功能,降低疼痛评分,提高患者生活质量,安全可靠。
Objective To investigate the therapeutic effects of interlocking intramedullary nailing internal fixation and locking plate internal fixation in treatment of type 2 diabetes mellitus complicated by intertrochanteric comminuted fracture of femur.Methods Seventy patients with type 2 diabetes mellitus complicated by intertrochanteric comminuted fracture of femur who were admitted and treated in our hospital from January 1,2017 to January 1,2018 were enrolled in the study,who were randomly divided into two groups according to random number table method.The patients in group A were treated by interlocking intramedullary nailing internal fixation,however,the patients in group B wer etreated by locked plate internal fixation.The surgical indexes,good/excellent rate of therapeutic effects,hip function,postoperative pain score at different time points after surgery,life quality score at one year after operation,and adverse events were observed and compared between the two groups.Results The operation time,intraoperative blood loss,postoperative drainage volume and fracture healing time in group A were significantly less than those in group B,moreover,the good/excellent rates of therapeutic effects in group A were significantly higher than those in group B(P<0.05).There was no significant difference in the good/excellent rates of hip joint recovery between the two groups(P>0.05).The postoperative VAS scores in both groups were significantly decreased,however,which in group A were superior to those in group B(P<0.05).Moreover the physiological function,physiological function,physical pain,vitality,social function,emotional function,mental health and overall health status scores in both groups were significantly improved after treatment,which in group B were superior to those in group A(P<0.05).However there was no significant difference in the incidence rate of adverse events between the two groups at 6 months after surgery(P>0.05).Conclusion As compared with locking plate internal fixation,the interlocking intramedullary n
