摘要
目的分析丙泊酚配伍小剂量芬太尼静脉麻醉用于胃镜检查镇静镇痛的临床效果。方法回顾性选择丙泊酚配伍小剂量芬太尼静脉麻醉用于无痛胃镜检查6010例设为试验组;同期普通胃镜检查917例设为对照组。试验组在利多卡因胶浆表面麻醉后,以丙泊酚配伍小剂量芬太尼静脉麻醉行无痛胃镜检查,对照组仅行利多卡因胶浆表面麻醉。比较两组患者镇痛镇静程度、HR、SBP、DBP和SpO2。结果试验组5799例(96.49%)患者对镇痛镇静效果肯定211例(3.51%)有肢动需要适量追加丙泊酚,HR、SBP、DBP和SpO2稳定。对照组多有恶心、呕吐、挣扎等不适和HR、SBP、DBP变化,组间HR、SBP、DBP比较差异有统计学意义(P<0.05)。结论丙泊酚配伍小剂量芬太尼静脉麻醉下行胃镜检查镇痛镇静效果确切患者舒适合作,可满意消除检查对机体的应激反应。
Objective To analyses the clinical effect of the intravenous anesthesia with propofol and combined low-dose fentanyl in gastroscopy for sedation and analgesia.Methods 6010 cases of painless gastroscopy under propofol combined with low-dose fentanyl were enrolled into the test group retrospectively,meanwhile,917 cases of regular gastroscopy in the same period were absorbed into the control group.After the surface anesthesia with lidocaine mucilage gases in the test group accepted painless gastroscopy under the intravenous anesthesia with propofol and combined low-dose fentanyl.While patients in the control group only received surface anesthesia with lidocaine mucilage.Analgesic and sedative degree,HR,SBP,DBP and SpO2 were observed between two groups.Results In the test group,exact analgesic and sedative effects were observed in 5799 cases(96.49%),211 cases(3.51%) showed body movements which need add propofol appropriately.The HR,SBP,DBP and SpO2 were stable.In the control group,discomforts such as nausea,emesis,struggle,etc.and changes in HR,SBP,DBP were observed.The HR,SBP and DBP between two groups showed significant differences(P<0.05).Conclusion The analgesic and sedative effects of propofol combined with low-dose fentanyl in gastroscopy were exact.Patients felt comfortable and cooperated well.The stress response of the body to the examination could be eliminated.
