摘要
目的探讨综合治疗对结节性痒疹患者生活质量及与人血清P物质水平的关系。方法选取2015年3月至2018年6月结节性痒疹患者200例,依据随机数字表法分为2组,每组100例。对照组予曲安奈德新霉素贴膏联合卤米松软膏外用及盐酸依匹斯汀胶囊口服,治疗组在对照组基础上给予患者心理干预,治疗期15 d为1个疗程,共4个疗程,治疗结束后随访半年,分别观察2组患者治疗前后及随访结束后皮肤病生活质量指数(DLQI)、临床疗效、汉密顿抑郁量表(HAMD)、人血清P物质、白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)等比较。结果治疗后2组患者上述症状均较治疗前好转,其中治疗组各指标评价疗效均明显优于对照组患者(P<0.05),2组患者人血清P物质、IL-6、TNF-α、生活质量均较治疗前好转,其中治疗组上述指标均明显优于对照组(P<0.05),随访结束后,治疗组患者指标较治疗后继续改善,差异均有统计学意义(P<0.05)。结论综合治疗对患者生活指数、临床疗效、抑郁焦虑情况、人血清P物质、IL-6、TNF-α、生活质量均有明显改善作用,且其半年随访后治疗组患者的上述指标也呈现出明显好转趋势,说明两种手段联合应用可有效延长结节性痒疹患者的临床痊愈期,改善患者的临床症状,值得在临床中进一步推广应用。
Objective To investigate the effects of epinastine hydrochloride capsulesc combined with mental intervention on the life quality and serum levels of substance P in patients with nodular pruritus.Methods A total of 200 patients with nodular prurigo who were treated in our hospital from March 2013 to December 2016 were enrolled in the study,who were divided into treatment group(n=100)and control group(n=100)according to random number table.The patients in control group were treated by triamcinolone acetonide neomycin ointment combined with halometasone ointment for external use and epinastine hydrochloride capsule for oral use,however,the patients in treatment group,on the basis of control group,were treated by psychological intervention,with a treatment course of 15 days for both groups,and a total of four courses of treatment,with 6-month follow up after teatment.The skin disease quality of life index(DLQI),clinical efficacy,Hamilton Depression Scale(HAMD),human serum substance P,interleukin-6(IL-6),tumor necrosis factor-alpha(TNF-α)before and after treatment were observed and compared between the two groups.Results After treatment,the above indexes were obviously improved in both groups,moreover,which in treatment group were superior to those in control group(P<0.05).The serum substance P,IL-6,TNF-α,life quality of patients were significantly improved in both groups,moreover,which in treatment group were superior to those in control group(P<0.05).Conclusion The comprehensive treatment can significantly improve the life index,clinical efficacy,depression and anxiety status,substance P in human serum,IL-6,TNF-αand life quality of patients with nodular prurigo,which can effectively prolong the clinical recovery period of patients and improve the clinical symptoms of patients.Therefor it is worthy of further popularization and application in clinical practice.
