摘要
目的探讨短节段固定融合术治疗退变性腰椎侧凸(DLS)合并腰椎管狭窄的临床效果。方法选取我院2015年3月5日至2017年3月24日收治的76例DLS合并腰椎管狭窄患者作为研究对象,采用分层随机法分为观察组(38例,短节段固定融合术)与对照组(38例,单纯椎管减压术)。比较两组患者的治疗效果。结果治疗后3个月,两组的矢状面平衡、冠状面平衡、Cobb角均减小,且观察组明显小于对照组(P<0.05)。治疗后3个月,两组的VAS、ODI评分均降低,JOA评分均升高,且观察组明显优于对照组(P<0.05)。观察组的出血量明显少于对照组,腰痛症状改善时间及手术时间均明显短于对照组(P<0.05)。观察组的并发症总发生率及2年内死亡率均明显低于对照组(P<0.05)。结论短节段固定融合术治疗DLS合并腰椎管狭窄的临床效果显著,可加快患者恢复进展,减少其并发症发生情况,值得临床推广应用。
Objective To investigate the clinical effect of short segmental fixation on degenerative lumbar scoliosis(DLS)with lumbar spinal stenosis. Methods A total of 76 patients with DLS and lumbar spinal stenosis admitted in our hospital from March 5, 2015 to March 24, 2017 were selected as the study objects, and the patients were divided into observation group(38 cases, short segmental fixation) and control group(38 cases, simple spinal decompression) by stratified random method. The therapeutic effects of the two groups were compared. Results Three months after treatment, the sagittal plane balance, coronal plane balance and Cobb angle of the two groups reduced, and those in the observation group were significantly smaller than the control group(P<0.05). Three months after treatment, the VAS and ODI scores of the two groups decreased, JOA scores increased, and those in the observation group were significantly better than the control group(P<0.05). The amount of bleeding in the observation group was significantly less than that in the control group, and the improvement time of lumbago symptoms and operation time were significantly shorter than those in the control group(P<0.05). The total incidence of complications and 2-year mortality in the observation group were significantly lower than those in the control group(P<0.05). Conclusion Short segmental fixation on DLS with lumbar spinal stenosis has a significant clinical effect, it can speed up the recovery progress of patients and reduce the incidence of complications, which is worthy of clinical promotion and application.
