摘要
目的:探究整体性护理对慢性阻塞性肺疾病(COPD)并呼吸衰竭老年患者排痰效果及血气指标的影响.方法:选取2018年1月-2019年1月笔者所在医院呼吸科收治的老年COPD并呼吸衰竭患者80例作为研究对象,按照护理方法不同将其分为两组,每组40例.对照组行常规护理干预,观察组行整体性护理干预.比较两组排痰效果[每天排痰量、血氧饱和度(SpO2)恢复时间、痰鸣音评分]、血气指标[血氧分压(PaO2)、二氧化碳分压(PaCO2)、SpO2、pH值]、护理满意度.结果:干预后观察组每天排痰量、痰鸣音评分及SpO2恢复时间均优于对照组,差异有统计学意义(P<0.05).干预后,两组患者的PaO2、SpO2水平及pH值均明显升高,PaCO2水平显著降低(P<0.05),且观察组PaO2、SpO2水平及pH值明显高于对照组,PaCO2水平低于对照组,差异均有统计学意义(P<0.05).观察组护理总满意度为95.00%,对照组护理总满意度为72.50%,两组比较差异有统计学意义(P<0.05).结论:对COPD并呼吸衰竭老年患者采用整体性护理,能够提高排痰效果,改善血气指标,促进呼吸功能恢复,提高护理满意度,值得临床推广.
Objective:To explore the effect of holistic nursing on sputum excretion and blood gas index in elderly patients with chronic obstructive pulmonary disease(COPD)and respiratory failure.Method:A total of 80 elderly patients with COPD and respiratory failure admitted in the respiratory department of our hospital from January 2018 to January 2019 were selected as the study objects.They were divided into two groups according to different nursing methods,with 40 cases in each group.The control group received routine nursing intervention,and the observation group received holistic nursing intervention.The sputum excretion effect[daily sputum excretion volume,blood oxygen saturation(SpO2)recovery time,sputum sound score],blood gas index(blood oxygen partial pressure(PaO2),carbon dioxide partial pressure(PaCO2),SpO2,pH value)and nursing satisfaction were compared between the two groups.Result:After intervention,the daily sputum excretion volume,sputum sound score and SpO2 recovery time of the observation group were all better than the control group,the differences were statistically significant(P<0.05).After intervention,the levels of PaO2,SpO2 and pH in the two groups were significantly increased,and the PaCO2 level was significantly decreased(P<0.05).The levels of PaO2,SpO2 and pH in the observation group were significantly higher than those in the control group,while the level of PaCO2 was lower than that in the control group,the differences were statistically significant(P<0.05).The total satisfaction of nursing in the observation group was 95.00%,and 72.50%in the control group.The difference between the two groups was statistically significant(P<0.05).Conclusion:Holistic nursing for elderly patients with COPD and respiratory failure can improve the effect of sputum excretion,improve blood gas index,promote respiratory function recovery,and improve nursing satisfaction,which is worthy of clinical promotion.
